Description
This is a Pre-Solicitation Notice being released for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) Part 5.2. This Pre-Solicitation Notice is issued to inform prospective Contractors of the intent of the Department of Veterans Affairs, VISN 4 Contracting Activity to negotiate a commercial item (FAR Part 12) contract for Pulmonary Services on behalf of the beneficiaries of the VA Medical Center, Erie, PA. Contractors are advised that the United States (U.S) Government will not pay for any information or administrative cost incurred in response to this Pre-Solicitation Notice. All costs associated with responding to this Pre-Solicitation Notice will be solely at the responding party's expense. Please be advised that all submissions become Government property and will not be returned. If a Request for Proposal (RFP) is released, it will be issued via the Federal Business Opportunities website (www.fbo.gov) and will be a competitive small business set-aside for a base plus 4 twelve month options contact. The information provided in this Pre-Solicitation Notice is subject to change and is not binding on the Department of Veterans Affairs. It is the responsibility of the potential Contractors to monitor this website for any information or changes that may pertain to this Pre-Solicitation Notice or a future RFP. All Offerors must be registered in System for Award Management (SAM) under NAICS 622110, size standard $10 Million to be eligible for award. Personnel assigned by the Contractor to perform the services covered by this contract shall be citizens of the United States. Pulmonary patient care services will include patient assessment, reassessment, treatment protocols, and patient management. Patient management will include but is not limited to outpatient clinics and inpatient consultations. The pulmonologist will provide professional reads for pulmonary function testing, as needed. Physicians must have a current, full and unrestricted license to practice medicine in a State, Territory, or Commonwealth of the United States or the District of Columbia. Physicians appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C 7402(d). The qualifications of such personnel shall also be subject for review by the Chief of Staff. It is preferred that the designated physician(s) possess Board certification by the American Board of Medical Specialties (ABMS), or the Bureau of Osteopathic Specialties, or be Board eligible within 4 years of residency or fellowship completion. The physician's qualifications will be subject to the credentialing and privileging process by the Erie VA Medical Center (EVAMC). The physician will also be subject to Erie VAMC's requirement for VetPro Updated licensing, registration and certification will be provided as they are renewed. Contractor will be responsible for providing their Physician's National Provider Identifier numbers (NPI). The contractor is responsible for and shall maintain all Federal and State licensing and certification requirements. When requested by the Contracting Officer, or authorized representative, the Contractor shall provide the EVAMC Credentialing Coordinator with the required completed EVAMC credentialing and privileging application forms (and VetPro requirements) that include information pertaining to documentation of education and training, experience, license and references. The Contractor shall adhere to and comply with any and all Joint Commission (JC) or other Accreditation Board requirements, including maintenance of records that document the provider's competence/performance improvement in accordance with EVAMC and JC or other Accreditation Board requirements.
