- ICA WATER STUDY STATEMENT OF WORK A. BACKGROUND In 2007, a 7.9 magnitude earthquake hit the Ica department of Peru, resulting in the collapse of the existing water and sanitation system. An estimated 75% of households in Ica are currently serviced by piped in-home connections, but there is evidence that only 32% of households have microbiologically safe drinking water sources. In response to the current water and sanitation situation and chronic earthquake-related risk of damage to infrastructure in Ica, the U.S. Naval Medical Research Unit - 6 (NAMRU-6) in Lima, Peru, in collaboration with the U.S. Peace Corps, Ica regional health authorities, and the University of Washington, seeks to conduct research to improve the quality of household drinking water in the region. NAMRU-6 has been present in Peru since 1983 and, in cooperation with the Peruvian Navy, has the objective of conducting research and surveillance in infectious diseases that affect human health in Central and South America. The proposed work will take place in one of the rural sub-districts of the Pisco district, in the Ica region, where U.S. Peace Corps water and sanitation volunteers have had a presence over the past 5 years. Although approximately half of the population in this sub-district is served by improved (piped) water sources, sources may be inconsistently or inadequately treated and therefore the population is at risk of consuming microbiologically contaminated water. Given the limited resources of the local health authorities to perform consistent monitoring of district water supplies, particularly in outlying communities that lack access to improved water sources, this work will aim to complement their current work and to recommend measures to protect communities at risk of waterborne disease. B. OBJECTIVES • Implement the study protocol to achieve the following aims: 1) Measure the prevalence and identify the risk factors for contamination of household drinking water supplies in the region 2) Evaluate the impact of low-cost interventions in improving household drinking water quality • Collaborate with U.S. Peace Corps volunteers in the implementation of study activities • Collaborate with local health authorities in the implementation of study activities and to share knowledge C. SCOPE OF WORK Implementation of Study Protocol Contractor will be required to: • Hire a coordinator to: 1) Perform the following activities: - Complete human subjects training - Lead the hiring of field personnel - Arrange for the appropriate training of study personnel for the conduct of human subjects research, collaborating with NAMRU-6 to ensure that documentation of said training is provided to the relevant research ethics committees - Conduct the training of study field workers to ensure their adequate preparation for the implementation of the protocol (training in study procedures) - Supervision of field personnel to ensure that activities are accomplished in accordance with the protocol - Perform survey quality control measures to ensure data quality, as per study protocol - Submit monthly progress reports to NAMRU-6 point of contact 2) The coordinator should have the following characteristics: - University degree preferred - Experience leading and supervising the implementation of public health projects in the field, preferably with experience supervising epidemiologic studies - Experience leading human subjects training sessions for study workers preferred - Be responsible and detail-oriented - Preferably living in or near the study area • Hire personnel to: 1) Perform the following activities: - Complete human subjects training prior to start of all study activities - Conduct study census - Complete training in study procedures, including completion of a pilot study - Perform informed consent with subjects - Conduct study surveys, including chlorine testing of water samples and collection of water samples for microbiological testing - Process water samples and record water sampling data - Enter data into database 2) Personnel should have the following characteristics: - High school degree preferred - Be responsible and detail-oriented - Preferably with previous experience in field surveys and microbiologic testing of water samples - Preferably living in the study area • Coordinate with NAMRU-6 the following: - Start date of activities - Transport of selected supplies previously purchased by NAMRU-6 - Purchase and transport of selected supplies - Travel of co-investigators to assist in personnel training and to perform monthly monitoring visits during the main study period • Arrange for the transport of study field workers and materials to subject households • Collaborate with local health authorities and U.S. Peace Corps volunteers to establish a study office in or near the study area • Perform the following fieldwork activities (led by study coordinator) 1) Census of the area in accordance with the protocol - Develop a work schedule for the census activities - Identify study-eligible households from the approximately 2,000 households in the region and recording locator information (using GPS) from eligible households (approximately 500) to enable later enrollment in the research study - Obtain printed materials, clipboards, pens, backpacks, and any other supplies necessary for the completion of the census - Enter data on a daily basis, with weekly data transmissions to designated study investigators - Make arrangements to enable communication among all study personnel and with study investigators in Lima 2) Conduct a pilot study (2 weeks) in order to: a. Test the feasibility of study procedures among a group of approximately 30 households b. Complement classroom training of field workers in study procedures 3) Perform the proposed main study in accordance with the details of the protocol (14 weeks) - Conduct baseline and six follow-up visits involving surveys and water sampling among approximately 210 households - Conduct appropriate quality control measures for survey and water sampling data as per the study protocol - Enter data on a daily basis and transmit to designated investigators on a weekly basis - Communicate with investigators to resolve any difficulties encountered in the course of data collection - Monitor study supply stock 4) Ensure accomplishment of end-of-study activities - Return any excess supply to NAMRU-6 - Return of paper records to NAMRU-6 Data Center for storage Collaboration with the U.S. Peace Corps • Involve U.S. Peace Corps volunteers in the implementation in all study activities, or as possible given other commitments of the volunteer Collaboration with local health authorities • Collaborate with local health authorities to ensure that the logistical needs of the completion of study procedures are met and to share knowledge gained through the course of this study D. TECHNICAL REQUIREMENTS In general: • NAMRU-6 will provide direct technical assistance in the implementation of the project and provide selected study supplies. • Contractor will have experience in the performance of projects in the water and sanitation sector and in the conduct of epidemiologic field studies. • Contractor will hire personnel as required to carry out the tasks described in the Scope of Work. • Contractor will provide services using their own resources, equipment, tools, etc. if not provided by NAMRU-6, and they must have the necessary infrastructure to adequately provide the services agreed upon. • Contractor will provide services from their own offices or from a location that enables them to meet the obligations of this contract. • Contractor commits to make all reasonable efforts and to put all their experience and knowledge for effective and efficient performance of contracted services. E. TRAVEL Travel under this contract is subject to allowances authorized in accordance with the Contractor's Institutional Travel Regulations (if any). F. PAYMENT Payment will be effected monthly upon receipt of a proper Invoice from the Contractor. Payment will be processed at the end of each corresponding month, and will be deposited directly in the Contractor's account via wire transfer.
