- This is a combined synopsis/solicitation for water testing for the dialysis department at the VAPIHCS in Honolulu, Hawaii. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All quotes must be submitted not later than May 17, 2013 at 8:00 AM HST to Nicole.do@va.gov. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Price: Contractors shall submit quotes on the worksheet attached. 2.Technical: Contractors shall submit proof of knowledge and experience in conducting LAL Assay Endotoxin testing in accordance with AAMI standards. Examples of sufficient proof include: resumes, past performance, certifications, and/or company profile detailing extent of this type of testing. Technical is more important than price. This is a 100% small business set aside, NAICS code 541380 (size standard: $14 million). Performance Work Statement Water Sampling for Veterans Affairs Dialysis Unit Site:VA Pacific Islands Healthcare System 459 Patterson Road Honolulu, HI 96819 Technical POC:Myrna Acson-Saldevar, Nurse Manager 1.0 Background 1.1 The intent of this Performance Work Statement (PWS) is to contract with a laboratory to provide monthly and follow-up testing of water and dialysate samples for endotoxin levels, expressed as EU/mL (endotoxin units per ml). 1.2 Monitoring and testing of both water and dialysate is necessary to assure that both are safe for patient use in hemodialysis. A patient on hemodialysis is exposed to anywhere from 90-190 liters of water in the dialysate per treatment. Exposure to endotoxins in the water can cause an acute pyrogenic reaction, with possible temperature spike, chills, rigors, and hypotension. 2.0 Requirement 2.1 Endotoxin testing is done using the Limulus amoebocyte lysate (LAL) assay. In accordance with standards established by AAMI (Association for the Advancement of Medical Instrumentation), endotoxin in the water used for hemodialysis should not exceed 2 EU/ml, and action must be taken when the level exceeds 1 EU/mL. If the action level is exceeded, corrective steps should be taken by the VA staff such as disinfection of the dialysis machine and the water samples will be re-submitted for testing, until the endotoxin levels are within acceptable limits. 2.2 The VA Pacific Islands Health Care System (VAPIHCS) expects approximately eight LAL Assay Endotoxin testings per month. 2.3 Each month, the contractor shall pick up the eight water samples from the VAPIHCS. Testing of the water shall be completed and returned to the VAPIHCS (c/o Myrna Acson-Saldevar) no later than five days after pick up. 2.4 Each test reading shall display the endotoxin levels for each test. 3.0 Requirements for quality assurance: "General liability insurance for the contractor is needed. "All VA, TAMC, Joint Commission, and AAMI requirements must be met, including credentialing of appropriate contractor staff. "All liability for contractor-provided services is assumed by the contractor. "As needed, the contractor will support quality improvement initiatives of the VA Dialysis Unit.
