- This is a combined synopsis/solicitation for commercial services prepared in accordance with Federal Acquisition Regulations (FAR) Subpart 12.6, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. This solicitation document, and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-54, The FAC, is available at https://www.acquisition.gov/FAR. This procurement is 100% set-aside for small businesses under NAICS code 621999. The companys System for Award Management (SAM, www.SAM.gov) record must be current, and indicate that the entity is considered a small business. The vendor to which this contract is awarded must also be registered in, and submit invoices through, the Invoice Processing Platform (www.IPP.gov). The resulting award will be a firm fixed-priced contract with economic price adjustment, with one base year and four optional years. The National Park Service (NPS) exercises its right to extend the contract every 12 months for the next 12 months in accordance with the vendors quote in response to this solicitation. The contractor must accept & perform in accordance with the extended contract and prices listed in the proposal. The anticipated award date for this contract is 01 June 2013. The base year of the contract shall be from the date of award through the next 12 months. The National Park Service has a requirement to conduct audiometric testing at several parks around northern Arizona in accordance with the scope of work below. Companies are welcome to use their own format to provide a bid, or the provided standard form 1449, as long as each years pricing are individual line items and sales tax, if applicable, is listed separately. This contract is subject to Service Contract Act Wage Rate WD 05-2023 (Rev.-16) first posted on www.wdol.gov on 06/19/2012. SCOPE OF SERVICES 1.This contract is to provide audiograms to National Park Service employees from the following locations: Grand Canyon National Park, North and South Rims; Flagstaff Area Monuments Parks, Sunset Crater National Monument, Walnut Canyon National Monument, Wupatki National Monument; the Southern Four Corners Group, Hubble Trading Post National Historic Site, Canyon De Chelly National Monument, Navajo National Monument; and Camp Verde Parks, Montezuma Castle National Monument, Montezuma Well National Monument, and Tuzigoot National Monument. 2.The contractor shall provide appropriate facilities for conducting the audiometric testing. The contractor may on a case by case basis request the park provide a room appropriate to conduct the audiometric testing. 3.The duration of this contract shall be for one base year plus four optional years. 4.Contractor certifications shall comply with 29CFR1910.95(g)(3). Contractor shall provide this information with bid submission. 5.The contractor will provide audiometric testing that complies with the 29CFR1910.95(h). a.Audiometric tests shall be pure tone, air conduction, hearing threshold examinations, with test frequencies including as a minimum 500, 1000, 2000, 3000, 4000, and 6000 Hz. b.Tests at each frequency shall be taken separately for each ear. 6.Audiometric tests shall be conducted in accordance with American National Standard Specification for Audiometers, S3.6-1969. 7.Audiometer calibration shall be checked acoustically at least annually in accordance with Appendix E: "Acoustic Calibration of Audiometers." Test frequencies below 500 Hz and above 6000 Hz may be omitted from this check. Deviations of 15 decibels or greater require an exhaustive calibration. 8.An exhaustive calibration shall be performed at least every two years in accordance with sections 4.1.2; 4.1.3.; 4.1.4.3; 4.2; 4.4.1; 4.4.2; 4.4.3; and 4.5 of the American National Standard Specification for Audiometers, S3.6-1969. 9.The contractor shall have the most current equipment calibration records available for inspection by the National Park Service representative at the time of the audiometric testing. 10.Audiometric examinations shall be administered in a room meeting the requirements listed in 29CFR1910.95 Appendix D: "Audiometric Test Rooms." 11.The contractor shall use existing records for the employee baseline. For employees new to the hearing conservation program, the contractor shall establish the employees baseline. 12.The contractor shall provide audiometric testing at the following locations: a.Grand Canyon National Park south rim maintenance area; b.Grand Canyon National Park north rim maintenance area; c.Sunset Crater Volcano National Monument maintenance area; d.Montezuma Castle National Monument maintenance area. e.Canyon De Chelly National Monument maintenance area. 13.110v and 220v electrical power is available at the specified. 14.The contractor shall provide audiogram packets to the employee which include at a minimum: a.Results of audiometric testing, b.Recommendations for follow up actions based on test results, c.If audiometric testing indicates that a Significant Threshold Shift (STS) has occurred, i.A Written notice of a STS, ii.A STS Counseling Checklist that describes steps to be taken after a STS is identified. 1.This checklist, if followed, will ensure compliance with OSHA requirements. 2.This checklist will be providing in English or in Spanish at the employees request. d.Appropriate training materials to help employees better understand the relationship between hearing and noise, the physiology of hearing, what hearing loss means in terms of what can and cannot be heard due to hearing loss, etc. 15.The contractor shall provide audiometric testing booklets for record keeping purposes with a distribution as follows: a.Camp Verde Monuments including Montezuma Castle National Monument, Montezuma Well National Monument, and Tuzigoot National Monument will receive a booklet containing audiometric test results from employees from all three parks. b.Flagstaff Area Monuments including Sunset Crater National Monument, Walnut Canyon National Monument, Wupatki National Monument will receive a booklet containing audiometric test results from employees from all three parks. c.Navajo National Monument will receive a booklet containing audiometric test results from its employees. d.Canyon De Chelly National Monument will receive a booklet containing audiometric test results from its employees. e.Hubble Trading Post National Historic Site will receive a booklet containing audiometric test results from its employees. f.Grand Canyon National Park Safety Office will receive a booklet containing audiometric test results from employees including both north and south rims. 16.The content of the audiometric testing booklets shall contain at a minimum: a.A list and description of forms used, b.A list of employees who have suffered a STS. c.A list of employees who have a normal hearing range, d.A list of employees who have an overall loss of hearing, unilateral or bilateral, e.A list of employees who have a mild to moderate high frequency hearing loss with no involvement in the speech range, f.A list of employees who have a mild to moderate high frequency hearing loss with involvement in the speech range, g.A list of employees who have severe high frequency hearing loss with no involvement in the speech range, h.A list of employees who have severe high frequency hearing loss with involvement in the speech range, i.A list of employees who should see a doctor unless seen within the last 12 months, j.A list of employees who have hearing loss at one pitch, k.A list of employees who are referred to an audiologist, l.A list of employees who are referred to an audiologist due to STS. m.A copy of the STS notice. n.A copy of the STS Counseling Checklist that is given to employees whose test results indicate a STS. o.A copy of the referral letter advising the employee to seek an examination by an audiologist or a physician specializing in ear, nose, and throat to have their hearing evaluated.
