- The purpose of this sources sought is to identify potential sources to provide cardiac wellness services for approximately 65 emergency services personnel to include personalized fitness, nutrition, and medical assessment to be provided by health professionals that are qualified to evaluate and manage the health and fitness needs. An initial nutritional and fitness assessment of each participant would be performed at the time of enrollment. This assessment would be conducted in facilities at Sierra Army Depot (SIAD) located in Herlong California. The contractor shall also have a facility located within commuting distance (90 miles) of SIAD so personnel could be seen at the office. The requirement would also involve follow-on visits provided to participants based on health risk evaluation results. These would occur at SIAD or a contractors' facility located within commuting distance to SIAD and would consist of individualized nutrition and fitness counseling with functional improvements monitored through periodic interviews with the participants. The contractor would need to be able to provide follow-on visits using a secure web-based health risk assessment (HRA), customized to evaluate emergency services personnel and would be available to each participant as well as the results of the individual assessments would be made available to SIAD occupational health through a secure online access. The contractor would provide Lipid NMR, Lpa testing and Carotid IMA testing for each participant upon enrollment and periodically as determined by HRA results with the results interpreted by a qualified physician. The requirement would also involve risk analysis and data management to be provided by the contract agency with all information available to each individual and SIAD occupational health through a secure online database. The contractor would also be required to provide educational videos and materials to address the specific health requirements of emergency services personnel. Qualified Occupational Medicine physicians would need to be available for consult in coordination with nutritional and fitness counselors. These professionals will provide guidance to participants and feedback to SIAD occupational health. This notice shall not be construed as a Request For Proposal, Request For Quote, Invitation For Bid, or as an obligation on the part of the Government to acquire follow-on acquisitions, however the Government may at its discretion award a contract(s) on the basis of this Notice, but is in no way obligated to pay for the information requested. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of submission of responses to this notice and Government use of such information. The Government recognizes that proprietary data may be part of this effort. If so included, clearly mark such restricted or proprietary data and present it as an addendum to the non-restricted/non-proprietary information. NO SOLICITATION DOCUMENT EXISTS AT THIS TIME, however the government will review responses to identify firms potentially capable of fulfilling the Governments requirements. A response to this notice in no way obligates the government to enter into a potential contract.
