Description
This is a special notice, not a request for either a quote, proposal, or an invitation for bid. The intent of this notice is to obtain industry comments and knowledge for the purpose of assessing the availability sources. The 5th Contracting Squadron, Minot AFB ND is seeking sources for tobacco cessation quitline services. The NAICS code is 813212. The small business size standard is $25.5M. Responses to this sources sought notice will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought notice, if the Government still plans to proceed with the acquisition, a solicitation announcement will be published on www.fbo.gov. No solicitation is currently available. It is the potential offeror's responsibility to monitor this site for future synopsis/solicitation as they will not be notified. The contractor would be responsible for providing labor and materials for a tobacco cessation telephone helpline for Minot AFB, ND active duty military personnel and their beneficiaries who are enrolled to the 5 MDG. Retired military and their dependents that are enrolled to the 5 MDG also qualify for the Helpline. Counseling for customers will begin when the individual calls the toll-free number. A tobacco cessation counselor will work with each individual to develop a detailed tobacco cessation plan best suited to that person's tobacco habits and lifestyle. Each caller will receive individualized counseling and a free packet of information outlining the tobacco cessation process. The contractor will mail the packet of information directly to the individual's home address. The contractor will not limit the number of calls. The client will be informed by the counselor to call the Helpline on a weekly basis to continue the counseling process. If a participant goes greater than 10 days without calling, they will be out of the program for a period of 3 months. The participant will be instructed by the contractor to make an appointment with his/her PCM if any NRT is recommended. PCM to prescribe all medication. Participant will call the contractor back regarding medications/treatment prescribed by PCM. Contractor to inform Active Duty members those NRT medications must be obtained through Minot AFB pharmacy in 5 MDG. Contractor is to instruct PRP/Flight members to pick up NRT on same day they drop pink slip off at the pharmacy. Reporting: After the initial call the contractor will send a secured email to the HAWC with the name and treatment plan selected for each client. The Contractor shall be required to forward both a telephone consult (t-Con) and a Subjective Objective Assessment Plan (SOAP) note after the initial and each follow up contact via secured email to the HAWC, for each individual who has requested to begin the smoking cessation process. The Health Educator from the HAWC will be the primary contact however the Exercise Physiologist and Registered Dietician shall be alternates. The contractor will be required to forward a quarterly report listing all clients who have participated in tobacco cessation counseling. The report will include name, date, number of minutes spent on calls, and type of tobacco used. The contractor shall provide service Monday through Friday, 0700 to 2300 CST. The contractor shall not provide services on Federal holidays: New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day. The contractor shall employ and maintain a technically trained and experienced work force. Counselors will be certified in tobacco cessation training, and shall conduct themselves in a professional, courteous manner.
