- ATTACHMENT 3 - QUESTION FORM ATTACHMENT 2 - SITE VISIT RESERVATION FORM ATTACHMENT 5 - PAST AND PRESENT PERFORMANCE QUESTIONNAIRE ATTACHMENT 1 - PERFORMANCE WORK STATEMENT ATTACHMENT 4 - PAST PRESENT PERFORMANCE FORM SOLICITATION The Air Force is soliciting for proposals from qualified vendors for a firm-fixed price contract for Japan Joint Land Mobile Radio Maintenance and Repair services, located at Kadena Air Base and various other Military installations on Okinawa and mainland Japan. The procurement is a commercial acquisition wherein the contractor shall provide all management, tools, supplies, equipment, labor, manpower, and appropriate license(s), qualification(s), and permissions necessary to perform and accomplish radio maintenance and repair services for the 18th Communications Squadron with contract service conducted at Kadena AB, and various other Military Installations on Okinawa, Japan. This requirement is a commercial acquisition using Commercial Acquisition procedures (Reference FAR Part 12) and will be negotiated pursuant to FAR Part 15. The proposed contract will be awarded using full and open competition with a period of performance anticipated at five (5) years; consisting of one (1) base year of twelve (12) months and four (4) option periods of twelve (12) months each. The expected contract start date will be 1 October 2013. The government intends to make a single award to the offeror that represents the best overall value considering technical, price, and past performance. Solicitation No. FA5270-13-R-0006 is being issued as a Request for Proposal (RFP) advertised with a North American Industry Classification System (NAICS) code of 811213. A site visit is scheduled for prospective offerors. Proposals will be accepted when submitted via e-mail to kyle.bookhardt@us.af.mil or when hand carried, proposals must be deposited in the bid depository located at 18 CONS/LGCA, Bldg 99, Kadena AB, Okinawa. Offerors must submit a paper copy of their proposal as well as all information required by the solicitation if proposal is submitted by email. The closing date for receipt of offers will be 17 May 2013 03:00AM Eastern Standard Time; 04:00PM Japan Standard Time. Anticipated award date will be on or around 30 August 2013. Facsimile, telephone or written request for the solicitation package will not be honored. HARD COPIES OF THE SOLICITATION WILL NOT BE ISSUED FOR ANY REASON. All prospective offerors interested in submitting proposals must have a Commercial and Governments Entity (CAGE) code, and must be registered in the System for Award Management (SAM) (https://www.sam.gov/portal/public/SAM/) in order to be eligible for receipt of contract award.
