- Statement of Work 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2. Solicitation number RFQ13-PHX-04 is being issued as a Request for Quote (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This solicitation is being issued under full and open competition. The North American Industry Classification System (NAICS) code is 484230 - Specialized Freight (except Used Goods) Trucking, Long-Distance, and the applicable size standard is $25,500,000.00. 5. This requirement consists of five (5) line items: 0001 - Waste Transportation Services, Supai 0002 - Option 1 Waste Transportation Services, Supai 0003 - Option 2 Waste Transportation Services, Supai 0004 - Option 3 Waste Transportation Services, Supai 0005 - Option 4 Waste Transportation Services, Supai 6. The Statement of Work is attached to this notification. The requirement is for the transportation of regular and bio-medic hazardous waste from the Havasupai Health Station in Supai, Arizona to the Peach Springs Health Center in Peach Springs, Arizona in accordance with the attached Statement of Work. 7. The location of contract performance is the Havasupai Health Station in Supai, Arizona 86435-0129, and the point of delivery is the Peach Springs Health Center in Peach Springs, Arizona 86434-0190. The Period of Performance will be 12 months from issuance of the contract with four (4) 12-month option periods. The total length of contract is will not exceed 5 years. 8.
