Description
This is a Request for Information (RFI) for Commercial products in accordance with the Federal Acquisition Regulation (FAR) 12.1. No proposals are requested by this announcement; all responses shall be solely in the form of information and materials. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. The Government is under no obligation to issue a solicitation or to award any contract on the basis of this RFI. Further, the Department of Veterans Affairs (VA) is not seeking proposals and will not accept unsolicited proposals or any cost or pricing data. The Government will not pay for any information or administrative costs incurred in response to this RFI. All costs associated with responding to this RFI will be the sole responsibility of the contractor. All submissions will be treated as business confidential materials, become Government property, and will not be returned. The Department of Veterans Affairs Network 15 intends to award a contract for a fully functional physiologic patient monitors system to include wireless telemetry. This requirement includes all equipment, labor, material and supplies necessary to provide this system for all the medical centers of VISN 15. The medical centers are Marion, IL; St. Louis, MO; Columbia, MO; Kansas City, MO; Leavenworth, KS; Topeka, KS; Poplar Bluff, MO; and Wichita, KS. The appropriate NAICS code is 334510 with a size standard of 500 employees. This includes the following Patient Monitoring System Requirements 1) Central Station 2) Central Station Client 3)OR monitors 4)PACU monitors 5)ICU monitors 6)Bedside monitors 7)Telemetry monitors 8)Portable monitors - non critical 9) Portable monitors - non-critical 10)Transport monitors - non critical 11)Transport monitors - critical 12)Telemetry Packs 13) Remote monitors14) Cabling/ Network 15) HL7 Solution 16) ECG Waveforms and VistA Imaging 17)Full disclosure 18)near real-time remote viewing solution 19) remote view for TeleICU and 20) Windows 7 and Citrix. In addition, the procurement includes clinical and biomedical engineering service training, installation and implementation services. Questions: (1) Is your company currently providing similar services to another government agency or other non-government customer? If so, please identify the agency or non-government customer. If you are unwilling to share your customers' identity, please address whether your company offers the same or similar services commercially (outside of the federal government). (2) Identify your company's Small Business size status based upon the applicable NAICS code of 334510. The Small Business Size Standard for this NAICS code is 500 employees. (4) Provide any other pertinent information as deemed necessary to demonstrate your and any proposed team member's capabilities and prior experience as it relates to the requirements set forth above. (5) Is your firm currently registered in VA's Vendor Information Pages (VIP) database? Yes ____ No ____ (6) List any General Services Administration (GSA) Schedules or Government-Wide Acquisition Contracts that you are currently performing under. Submittal Information The VA is looking to receive input from vendors capable of providing installing and implementing the physiologic patient monitor system with wireless telemetry. Interested parties may submit a capability statement, no more than five (5) pages, for your company. Please also denote the business size for North American Industry Classification System (NAICS) code 334510. Please include company name, company address, and DUNS Number.
