JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION DUE TO SOLE SOURCE REQUIREMENT COMPANY INFORMATION 1. FACILITY NUMBER-NAME - CITY, STATE: VA Ann Arbor Healthcare Center 2. VISN OR PROGRAM ACTIVITY: 11 3.POC NAME AND TELEPHONE NUMBER: Eric HenrIcks 734.678,3042 4.PROPOSED ACTION: Sole Source Procurement of SERVICE5.6.7.8.9.10.11.12.13.14.15. 16.DESCRIPTION OF REQUIREMENT: All studies will be performed on a Philips Achieve 3T system using an 8 channel receive head coll. Field homogeneity will be optimized up to 2nd order using field map based shimming software (Philips 'shimtool'). Single voxel MRS will be performed in 4 regions of Interest (ROM): (a) right anterior Insuta, (b) right posterior insole, (c) anterior cingulate cortex, and (d) right thalamus. These regions are chosen since they are based on prior imaging results implicating their involvement in pain processing. The MRI and MRS scanning protocol will consist of: 1) Survey images (1 minute scan time), 2) SENSE Reference images (2 minute scan time), 3) Axial 3D-MP-RAGE sequence (4:37 minutes). Isotropic 0,9 mm resolution, 150 slices, TRITE 8.2/3.7 msec, matrix 192x256, FOV 184 x 230, flip angle 8', SENSE factor (R) 2.0.17. NATURE & BACKGROUND OF ACQUISITION TO JUSTIFY SOLE SOURCE: The VA Ann Arbor HS does not have the equipment or expertise to perform the procedures required for this VA funded study.The 3-T MR.VMRS scanner that resides at the U of M Medical Center is required to gather data about the structure and chemistry of the brain. Specialized MR software and hardware, as well as appropriate physics/technical support are required for Image acquisition and post-processing and analysis of Images, which (hell of M Medical Center possesses. No other local source possesses this specialized equipment. "Describe the minimum salient characteristics that will meet your needs, This Is the area where you explain why no other vendor anywhere can supply this requirement for you. Acceptable reasons for 'sole source include (but are not limited to): bound by contract, technical order spectfication, warranty service. Of regional standardization. If the reason for sole source is determined locally, offer what market research or clinical guidelines led to the decision. If this item is included in an allowance standard for War Reserve Materiel (WRM), cite the allowance standard, required source, and stock number for this Item. NOTE FOR WRM PURCHASES: The Defense Logistics Agency is your primary source for WRM equipment. Be sure to determine availability from a DLA Item manager before contracting for WRM equipment. If DLA cannot meet your timelines for WRM equipment. your urgency may be further justification for other than full and open competition. See your Medical Logistics Flight Commander for more details. 8, PROVIDE INFORMATION SHOWING MARKET RESEARCH WAS CONDUCTED: The AAVAMC and the University of Michigan have an invaluable partnership. The University contains a state-of-the-art 3T MR scanner that can best acquire the protocol-specific scans for this initiatIve.The established partnership allows for the VA patients to be better served at the University than at any other local Institution. "This justification is for why no other vendor anywhere can supply your item or service and still meet your needs Your rationale for barring other vendors is crucial to your justification.
