- Sources sought synopsis announcement is issued to assist the 673d Contracting Squadron, JBER, Alaska, in performing market research to determine interest and capability. This is not a request for proposal (RFP) and responses to this request are not proposals and cannot be accepted by the Government to form a binding contract. This is not a guarantee of a future RFP or an award. This document should not be construed as a commitment of any kind by the Government. Any information provided to the Government in response to this document will become 673d Contracting Squadron property and will not be returned. The Government will not reimburse contractors for any cost associated with responding to this request. JBER, AK is seeking interested sources capable of performing preventive and corrective maintenance (on-site) for Bausch & Lomb Dtellaris BL11110 Cataract Extraction System JBER, AK. The Contractor shall assume all responsibility and liability personnel, equipment, tools, test equipment, transportation, parts, materials, supervision and other items and services necessary to perform preventive and corrective maintenance. The contractor shall provide a hardware and software, corrective and preventive maintenance program to include a complete set of special services tools, test equipment, fixtures, spare parts, verification procedures and schedules to provide an operational check of the system concurrent with normal operation and with no degradation in system performance, software problem analysis, associated reprogramming and correct software documentation. The contractor shall furnish and install all software updates and equipment modifications normally provided by the manufacturer, and installed at next PMI after they are made available by the manufacturer. The contractor shall have available any special tools, supplies, parts, transducers, and test equipment which are required for the servicing of the imaging systems. The contractor shall use only original or factory refurbished replacement parts and be quality assurance tested by the manufacturer. If required replacement parts are not immediately available to the contractor while performing Preventive Maintenance Inspections (PMIs) or service repair call, the contractor shall notify MERC personnel of the deficiency. The contractor's service personnel shall carry or have available in the local area the manufacturer's "recommended stock" for each system covered by this contract. Replacement parts shall be returned to the manufacturer, by the contractor. Maintenance actions shall not be delayed by more than 24 hours due to delivery of a repair part or item. All tools, test equipment, and supplies used for preventive and corrective maintenance are included in this contract. No charge will be made for them during the period of this contract. The contractor shall have and maintain a complete file of technical documentation on each system listed in the contract to include schematics, wiring diagrams, service manuals, troubleshooting guides, and service update memos. All equipment, parts, materials, and services supplied by the contractor shall conform to the applicable safety standards as indicated in the current code of Federal Regulations, Occupational Safety Heath Act (OSHA). The contractor shall furnish one time preventative maintenance support for the systems listed in attachment 1, ensure at least a 95% uptime. The contractor's maintenance service support shall provide all parts service, labor, travel time and expense to perform both preventive maintenance on all Bausch & Lomb Dtellaris BL11110 Cataract Extraction Systems. The period of performance is tentatively planned to last 5 working hours to begin 24 April 2013. The applicable North American Classification System (NAICS) code is 811219, with a business size of $19,000,000.00. Interested and qualified sources are requested to provide: (1) company name, point of contact, e-mail address, telephone and fax numbers, type and business size under the applicable NAICS code; (2) a list of the company's most recent and relevant (within the last three years) DoD or commercial contracts to include names, points of contact, phone numbers, value and type of contract, and brief description; and (3) information demonstrating the company's ability to perform these services. Respondents are asked to limit submissions to ten pages or less. Any information provided is voluntary. All eligible firms responding to this Sources Sought must ensure their response is complete and sufficiently detailed to allow the Government to determine the firm's qualifications to perform this requirement.
