Description
SOLE SOURCE JUSTIFICATION Indiana Blood Center Please note that this is neither a request for quotes nor a request for proposals. The Government intends to issue a contract on a Sole Source basis to Indiana Blood Center (IBC), to provide the blood products and laboratory services to the Roudebush Veterans Affair Medical Center. A specific listing of required supplies and services are listed below. Applicable NAICS code is 621991 size standard $10 million. The expected date of this award is June 1st 2013. Questions regarding this notice must be submitted in writing (email or fax is acceptable) and received no later than March 25th, 2013. No telephone inquires will be accepted. Faxes should be addressed to Felecia Beamen, Contract Specialist, at 317-988-1512; emails to felecia.beamen@va.gov. The Blood Center provides the following services: Products and Testing: blood banking services (to include autologous, directed, and homologous blood products and apheresis products) and laboratory testing services (to include donor testing, reference lab, HLA and paternity testing). Transportation: The Blood Center will provide a pickup service for specimens/products and a delivery service for products, upon request. All appropriate specimen containers and transportation materials will be provided by the Blood Center and remain the property of the Blood Center. Results reporting and Consultations: All specimens that are picked up by the Blood Center; shall be processed and the results returned, on a routine or emergency basis as requested. In addition, in the event any specimen registers a critical value, the Blood Center shall immediately convey such results to the appropriate Hospital personnel by telephone, facsimile or other electronic means. The Blood Center will make available consultative services for all tests performed for the Hospital. Consultations shall be performed upon the request of a Hospital pathologist, or referring physician staff members. (Within 15 miles) geographically located, to provide the appropriate products in a timely manner.
