- Sources Sought Description Document INTRODUCTION This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. PURPOSE The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are 8(a) small businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method; including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. The purpose of this acquisition is to provide FOH with a network of medical providers of occupational health medical services in Tallahassee, Florida. Physician(s) shall be currently board certified in any of the following: cardiology, emergency medicine, internal medicine, family medicine, or preventive medicine (aerospace, occupational, or general preventive medicine and public health) to qualify for performance of physical examinations under this contract. Other medical professional, i.e. optometrist, nurse practitioner, physician assistants and nurses shall have a current and unrestricted license to practice medicine in the jurisdiction of delivery of care. Staff certifications for Hearing Conservation by a CAOCH approved program are required for technicians conducting audiometric testing unless an audiometric microprocessor is used. The goal is to provide consistent standardized occupational health services in an area where FOH does not have a current presence in an efficient manner such that Federal employees do not have to travel long distances to receive services.
