This is the description of services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate Request for Proposal (RFP) will not be issued. The solicitation number is LGCAD-5-0272 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions are those in effect though Federal Acquisition Circular 2005-33 and DFARS change notice 20090715.. This NAICS code is 335911 and the size standard is 500 employees. Special Notes and Instructions: 1. Only SB vendors will be considered at this time. 2. This is a notice that this order is a total set-aside for a small business concern. Only quotes submitted by a small business concern will be accepted by the Government. Select appropriate set aside clause: 3. FAR 52.219-6 Notice of Total Small Business Set-Aside (NOV 2011). a. FAR 52.219-13 Notice of Set-Aside of Orders (NOV 2011). b. FAR 52.219-14 Limitations on Subcontracting (NOV 2011). 4. Must be IAW FAR 52.222-41 Service Contract Act 52.217-8 Option to Extend Services. OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days. The total evaluated price consists of the base and all options periods. In addition the option to Extend Services Clause ( FAR 52.217-8), is also considered part of this overall evaluation 52.217-9 Option to Extend the Term of the Contract. OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor no later than 15 days before the contract expires, provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 42 months. 5. Basis for Award. The 802nd CONS will issue a delivery order to the vendor who is determined to offer the best value to the Government. 6. Best Value Determination. The technically acceptable, lowest priced quote will be determined to represent the best value. Technical acceptability will be determined by an evaluation of the quoted requirements that meet the technical criteria that is contained in the solicited item description. If the lowest priced submission does not meet the technical criteria described in the solicited item description, the government reserves the right to evaluate the next lowest priced submissions until it has determined a technically acceptable submission. The evaluation will stop at the point when the government determines an offeror to be technically acceptable with the lowest evaluated price, because that offer will represent the best value. Contingent upon a determination of contractor responsibility, award will be made to that offeror without further evaluation of other offers. Award will be made to the responsible vendor whose quote is the lowest evaluated price among technically acceptable quotes. 7. Mandatory Registrations. To be eligible for award, the On-line Registration and Certifications Application (ORCA) and Central Contractor Registration (CCR) registrations must be current. If your Reps & Certs are not current go to the http: //orca.bpn.gov/ and provide mandatory information. For first time registration or to update CCR, go to https://www.sam.gov/portal/public/SAM/. Please include MFG name, Part Number, illustations, and literature in your RFQ submission when using ‘Equal' products. Details of the requesting product to fulfill requirement must be met. 9. Statement of Work. See attached document for details, and list of equipment 10. Notice to Offeror. The Government reserves the right to cancel this purchase request, either before or after suspense date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offereor for any costs. The Contractor shall not request, nor will the Government make, payments for any preliminary measurement, quotes, bids, site visit, time or proposal cost incurred. 11. Discussions. The government intends to award a purchase order without discussions with respective vendors/quoters. The government however, reserves the right to conduct discussions if deemed in its best interest. 12. Site Maintenance: If required to enter the operation center, all work areas shall be maintained in a neat, clean, safe condition and all trash, debris, and garbage, which the contractor generates shall be removed from the premises upon completion of repair, or daily if the completion/repair will be completed in more than one day. 13. Disposal of Unused product at the end of project: None 14. Completion of Work: The Contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all items/products in the description shall be fully operational at the completion of work for this project.
