- The Rock Island District, US Army Corps of Engineers is seeking qualified members of the Small Business Community - Small Business Disabled Small Business (SDB) Woman Owned Small Businesses (WOSB) Service Disabled Veteran Owned SB (SDVOSB) Historically Underutilized Business Zone (HubZones) and Section 8(a) Participants or small business concerns that qualify under the North American Industry Classification System (NAICS) Code 621399 - Offices of All Other Miscellaneous Health Practitioners for the following requirement located at Rock Island Clock Tower Safety Office, Rock Island IL The principal components of the work to be performed includes establishing a one year contract with the option of two additional years to staff an Occupation Health Nurse that is BOARD CERTIFIED or ELIBIBLE FOR BOARD CERTIFICATION by the American Board of Occupational Health Nurse in the Rock Island Clock Tower Safety Office. The requirement for the initial year is to update the Rock Island District Medical Surveillance Program to ensure correct examinations and screenings are conducted on District employees as required by OSHA, DoD, DA, and USACE. Additional year requirement is to provide medical Nursing Services. MEDICAL/PHYSICAL OVERSIGHT will also be a contract requirement that will be provided to the Occupational Health Nurse while on duty in the District. This announcement is part of our market research, and your responses are sought to identify sources that have the knowledge, skills, and capabilities to perform the services. Interested contractors, including small businesses that qualify under NAICS Code 621399 are hereby invited to submit a response to the market survey to demonstrate their technical, managerial, and business capability to provide the requested services. THIS SOURCES SOUGHT NOTICE DOES NOT CONSTITUTE A REQUEST FOR A FORMAL PROPOSAL. This notice is provided as information to the marketplace and is an invitation for an expression of interest and demonstration of small business capability to perform the anticipated work. The government will not pay for the provision of any information, nor will it compensate any respondents for the development of such information. All interested, capable, qualified, and responsive contractors are encouraged to reply to this MARKET SURVEY request. Interested Contractors must furnish the following information: GENERAL INFORMATION 1. What is your company name, address, point of contact, phone number, and e-mail address? 2. PAST PERFORMANCE INFORMATION Please provide any past performance information for the previous 3 years that clearly demonstrates familiarity and experience with the requirements. For each project, include the following information: a. Board Certified Health Nurse -Board Certified or Eligible for Board Certification b.Medical/Physician Oversight c.Size, term and complexity of the project. Explain your duties and the services you provided. What was the period of performance? d.Did you act as a Prime Contractor or a subcontractor? e.Provide point of contact information that is familiar with your services.
