- NEW VENDOR FORM W-9 FORM AFFIRMATION EEO NON-SEGRAGATED DAVIS BACON The South Bronx Job Corps Academy is a federally funded residential training facility for youth between the ages of 16 and 24. The program operates year round with 200 out of 300 trainees living on the premises during their enrollment. The Academy is operated for the United States Department of Labor by ResCare, Inc. The South Bronx Job Corps Center is accepting quotes for a Nurse Practitioner licensed by the state of New York to provide Subcontract Medical Services. These services will include working in conjunction with the center Physician in the development, implementation and monitoring of the total center health and wellness program and providing direct medical services to students. Approved candidate must provide malpractice insurance for medical services. This is a contract position that will be awarded for five (5) hours a week starting on April 1, 2013 through May 31, 2014. Nurse Practitioner Job Summary: Responsible for providing primary healthcare to patients, focusing on health maintenance, disease prevention, patient education and counseling; meets with patients to obtain a complete history; performs psychosocial assessments; assist in establishing the diagnosis; orders diagnostic evaluations; creates treatment plans; supervises; provides appropriate therapeutic and educational interventions; completes medication evaluations and discharge plans; functions as a mentor and clinical resource for other team members; performs other duties as assigned; inclusive of but not limited to: * Perform duties in accordance with the customary rules of ethics and conduct of the applicable State Board of Nursing and other such regulating bodies. * Prescribe dosages and instruct patients in correct usage. This includes uses of chemicals and their interactions, danger signs, production techniques, and disposal methods. * Respond to requests and inquiries from the general public in person and over the telephone. * Interpret the application of OSHA and HIPAA laws and adhere to them. Maintain quality, safety, and infection control standards. * Exercise tact and diplomacy in dealing with sensitive, complex and confidential patient issues and situations. * Establish and maintain effective working relationships with those contacted in the course of work. * Learn computer software programs; enter and maintain accurate data and statistical information in order to consistently prepare clear and concise reports, correspondence, and other written materials. * Directs patient care under the supervision of a physician and maintains effective medical provider - patient relationships. * Provides care in compliance with all Federal, state, local and corporate guidelines and in accordance with community standards of good practice. * Documents legibly, accurately and timely and adheres to all compliance requirements. Documentation will clearly reflect medical necessity, treatment provided, patient response to treatment, and correct coding of diagnosis and level of care. * Consults with collaborative physician as appropriate, carrying out physician orders and fully documenting the consult and actions taken in the medical chart. * Follows all appropriate safety and infection control measures when providing patient care. * Evaluates the physical, social and emotional needs of the patient and makes appropriate referrals to community resources. Actively coordinates care and communicates directly with others involved in the patient's care. * Involves the patient in the plan of care as allowed by current HIPAA regulations. * Demonstrates ability to manage case load effectively. * Demonstrates flexibility toward changing conditions. Requirements: -Current license as ARNP in New York. -Graduation from an approved Master of Science in Nursing Nurse Practitioner Program. -Satisfies the requirements of the New York Department of Professional Regulation Board of Nursing (Chapter 21 O-11). -Minimum of 2 years work experience as a Nurse Practitioner or five years RN nursing experience; Required Skills: -Neurologic diagnosis, medication protocols, interpersonal skills, written and verbal communication skills, customer service skills, critical- thinking, and organizational skills, time management skills, computer skill appropriate to position, administrative and clerical skills, work independently with minimal supervision.
