Description
The USPFO for NY is soliciting quotes for Medical Surveillance Examinations to be performed at various locations (outlined in the RFQ) throughout New York State for A Base Contract Period of 1 (One) year and four (4) option years to be exercised at the discretion of the Government. The estimated number of personnel and required exam/test services are also outlined in the RFQ. The purpose of the Occupational Medical Surveillance Examination Program is to provide job-related Medical Surveillance Examinations and Occupational Health Services to the NYARNG full-time support (FTS) and/or temporary personnel who are or could potentially be exposed to health hazards in the work environment. The examination shall include personnel assigned to positions requiring specific standards of physical fitness. Personnel not included under these examinations are State Employees, civilian contractors, and the part-time traditional Soldiers. The Government may evaluate the quality of professional and administrative services provided, but retain no control over the medical professional aspects of services rendered such as testing procedures, etc. The Contractor will provide Board Certified Occupational Health Physician(s), supervision, all labor, tools, materials, equipment, and incidentals required to conduct the Medical Surveillance Examination / Occupational Health Services for the NYARNG. The Statement of Work and Pricing sheet will be provided upon release of the RFQ. The expected date of release of the RFQ is MAR 11, 2013. The RFQ is set aside 100% for small business concerns. The NAICS Code applicable to this procurement is 621111, the small business size standard is $10,000,000. To be eligible for award, contractors must be actively registered in the System for Award Management (SAM). Go to https://www.sam.gov to register. The due date for quotes is 9 APR 2013 at 10:00 EST. Quotes may be faxed to (518)786-4985 or transmitted via e-mail to mark.r.walsh.civ@mail.mil The contractor bears the responsibility for the timely and accurate transmission of quote if utilizing fax or e-mail procedures. Questions concerning this procurement should be e-mailed to mark.r.walsh.civ@mail.mil. Answers to contractors questions will be uploaded periodically to the FBO website. Paper copies of the RFQ will not be issued. The RFQ and any and all amendments thereto will be uploaded to the FBO website for electronic downloading. Contractors are advised to check the FBO website frequently for any changes or amendments to the solicitation.
