Description
ATTACHED ARE CLAUSES FOR THE REQUISITION. Please return the SF18 with your bid. COMBINED SYNOPSIS/SOLICITATION WRAD-13-1262 500 TRAYS OF PRELOADED PIT TAGS CONTAINING 100 TAGS PER TRAY (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number WRAD-13-1262/WSB. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-65. (IV) This solicitation is opened to all small businesses. The associated NAICS code is 334220 with 750 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial item(s)/services: CLIN 0001: 500 Trays of preloaded client PIT tags containing 100 tags per tray. CLIN 0002: Shipping and handling of 500 trays of preloaded fish tags to Lower Granite Dam, Almota Road, Colfax, WA 99111. (VI) Description of requirements is as follows: STATEMENT OF NEED FOR TRAYS OF PRELOADED CLIENT PIT TAGS CONTAINING 100 TAGS PER TRAY AND PURPOSE: A preloaded tagging system is needed for use in 50,000 juvenile salmon that will traveling through Lower Granite Dam and other parts of the Columbia River in 2013. REQUIREMENTS: 500 trays of preloaded PIT tags are required containing 100 tags per tray. TECHNICAL SPECIFICATIONS: A tray is a packaging method that contains 100 loaded needle/tag assemblies positioned in a readily accessible manner. Each tag must be inserted into a clean 12-gage veterinary-grade needle assembly that includes a self-contained push-rod, a mechanism that will retain the tag in the needle, and a hub that can be inserted into an injection device. The preloaded tags must have a packaging system that will hold multiple loaded needles in a position such that they can be quickly inserted into an injection device. All loaded needles within an individual package must have the bevel in the same orientation. There must be a mechanism to hold or support the packaging system containing the loaded needles that allows free access to the needle hubs by the injection device. There must be an injection device capable of receiving the hub of the loaded needles that is rugged enough for several thousand uses. SHIPPING: All Pit Tags will be sent to the vendor and shipping paid by the customer. DELIVERABLE SCHEDULE: The preloaded tags are needed no later than April 5, 2013. End of Statement of Need (VII) Date(s) and place(s) of delivery and acceptance Date of Delivery: April 5, 2013 Delivery will be FOB Origin. (VIII) The period of performance for the base contract shall be from date of award to April 5, 2013. (IX) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun 2008), applies to this acquisition. In addition to written price quotes, offers are instructed to provide a capability statement describing qualifications to perform the requirements. Inquiries (Apr 2010). Offerors must submit all questions concerning this solicitation in writing to Bill Bell either through email Bill.Bell@noaa.gov or fax at 206 526-6025 no later than March 13, 2013 at 11am Pacific Standard Time. (X) FAR 52.212-2, Evaluation - Commercial Items 2005-65, applies to this acquisition. Paragraph (a) is hereby completed as follows: Lowest Price Technically Acceptable. Offers will be evaluated based on price and the factors set forth in the Section 6, past performance, FAR 12.601 and FAR 13.106-2, and award will be made to the firm offering the lowest price technically acceptable to the Government in accordance with the Statement of Need. (XI) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items 2005-65 and System for Award Management Registration (SAM) FAR Clause 52.204.99 (Class Deviation from 52.204-7 (September 2012) (DEVIATION). (XII) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items 2005-65, applies to this acquisition. (XIII) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items 2005-65 applies to this acquisition. The following clauses under subparagraph (b) apply: Please refer to the SF18 with Clauses. (XIV) The following clauses are also applicable to this acquisition: 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) 52.252-2 Clauses Incorporated By Reference (Feb 1998): This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
