- NOTICE OF PROPOSED CONTRACT ACTION SOURCES SOUGHT RFQ VA259-13-R-0398 The Sheridan, Wyoming VA Medical Center and Network Contracting Office (NCO 19) contemplates an award of a Firm Fixed Price contract for Fertilizing and Weed Control services resulting from a forthcoming solicitation. The proposed contract will be issued for a commercial firm fixed price contract type. It is anticipated this will be a 100% set aside for either a Service Disabled Veteran Owned Small Business (SDVOSB) or a Veteran Owned Small Business (VOSB) or a combination of both. The period of performance (PoP) for this contract will be one base year with additional four (4) one year option periods. Anticipated base year PoP is April 15, 2013 through April 14, 2014. This is a Sources Sought notice for fertilizing and weed control services. The contractor shall be able to provide all requirements stated in the below Performance Work Statement. In your email, please indicate the following: -Company Name -Point of Contact (POC) information -SDVOSB or VOSB -VETBIZ certified -DUNS number Please send your email to: Ricky Robins, Contracting Officer ricky.robins@va.gov 801-584-1243 PERFORMANCE WORK STATEMENT Fertilizer and Weed Control Sheridan VA Medical Center SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, and other items as necessary to fertilize and provide weed control at the Sheridan, Wyoming VA Health Care System, 1898 Fort Road, Sheridan Wyoming 82801. 1. TASK FREQUENCY AND INSTRUCTIONS a.FERTILIZE / WEED CONTROL Fertilize / Weed Control approximately 175 acres of lawn, 1100 trees and 30 bushes. The grounds are divided into six (6) sections. Application of fertilizer / weed control to all lawns, trees and bushes shall occur between the months of April to October of each year according to the following schedule. Area 1 - May Area 2 - July Area 3 - April (end of) Area 4 - May Area 5 - June Area 6 - August The contractor shall contact the Contracting Officer's Representative (COR) the month prior to application to coordinate the exact week within the month for fertilization / weed control. The Contractor shall ensure that correct soil conditions are maintained for each tree, shrub, flower, grass or plant in order to promote optimum growth, correct environment, and obvious good health for the species of plant, e.g., grass shall be of a lush, green color. Fertilizer / weed control used shall contain the appropriate nutrients and shall not be offensive to the smell. Fertilizer / weed control shall be delivered in the original, unopened containers bearing the manufacturer's chemical analysis, name, trade name, trademark, and indication of conformance to state and federal laws. In lieu of containers, fertilizer may be furnished in bulk if accompanied by a certificate indicating the above information with each delivery. b. ADDITIONAL WEED CONTROL TREATMENT The contractor shall provide additional weed control, as required, in the months of April through September. The COR will contact the contractor one week in advance to request additional weed control treatment requirements and what section of the grounds requires additional treatment. 2. PROPERTY DAMAGE The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc.) will be replaced in a timely manner and corrected by the contractor with like materials, at no extra cost to the government. 3. QUALITY CONTROL / ASSURANCE 3.1 QUALITY ASSURANCE SURVEILLANCE PLAN Performance ObjectiveSurveillance MethodPerformance Threshold The Contractor provides service within the mutually agreed one week time frame (with the exception of weather or natural disaster). 100% review No defects allowed. The Contractor shall ensure no damage to any government property. 100% review No defects allowed. 3.2 QUALITY CONTROL The Contractor is responsible for ensuring services are performed in accordance with the Performance Work Statement (PWS) requirements. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum the contractor shall develop quality control procedures addressing the areas identified in paragraph 3.1, Quality Assurance Surveillance Plan. 3.3 DOCUMENTING PERFORMANCE. The COR will verify performance of the contractor. The COR will document any deficiencies in writing and forward them to the contracting officer. 4. SAFETY The contractor shall provide to the COR all Material Safety Data Sheets (MSDS) of all products that the contractor will use in fertilizing / weed control.
