Description
Market Research for an inspection and survey to determine Life Safety code compliance (Life Safety Assessment) with the latest Edition of National Fire Protection Association (NFPA) Life Safety Code for the Jacksonville Outpatient Clinic located at 1526 Jefferson St., Jacksonville, FL 32209. This is a Sources Sought Notice only. This does not constitute a solicitation. The information gathered from this request for information (RFI) is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. The purpose of this notice is to identify mandatory sources of supply as required by FAR 8.002 and small business entities capable of performing an inspection and survey for the Jacksonville Outpatient Clinic located at 1526 Jefferson St., Jacksonville, FL 32209. Statement of Work 1.INTRODUCTION The North Florida South Georgia Veterans Health System has a requirement for an inspection and survey to determine Life Safety code compliance (Life Safety Assessment) with the latest Edition of National Fire Protection Association (NFPA) Life Safety Code. The Contractor shall provide all labor, tools, materials, equipment, apparatuses and supervision to perform the inspection and survey. This service is required for the new Jacksonville Outpatient Clinic (OPC). The address is 1536 Jefferson St., Jacksonville, FL 32209. This clinic is two stories high and 133,500 gross sq.ft; with 96,000 + usable net sq. ft. 2.BACKGROUND This Life Safety Assessment will be used to meet our requirements for Joint Commission accreditation and development of Plans for Improvement (PFI) as determined by NFSGVHS. 3.SCOPE The Contractor shall provide a Statement of Condition (SOC) including Life Safety Code Assessment (LSCA) in accordance with the latest edition of the National Fire Protection Association Life Safety Code; complete a Mock Environment of Care Survey, and updates Life Safety Drawings. 4.APPLICABLE DIRECTIVES None. 5.PERFORMANCE REQUIREMENTS a.Contractor shall provide inspection of the entire Jacksonville OPC, surveying for Life Safety Code compliance and non-compliance of the Ambulatory Healthcare Facility. b.Contractor shall develop a written report with a detailed list of each Life Safety Code deficiency including: a) unique identifier, b) locations of the deficiency, c) a description of the deficiency, d) recommended correction action and 3) the specific code reference. c.The contractor will submit the draft written report within twenty (20) business days after the site visit. The contractor shall fill in the identification portion of the plan for Improvement (PFI) short form in Part 4, leaving the resolution section blank for the facility to fill in. The report shall be submitted in an electronic format in Microsoft Word to the Medical Center Director and Chief Engineer/Facility Manager. d.Contractor shall provide all engineering and technical services, equipment and supplies to complete the Sate of Condition (SOC) survey in accordance with the NFPA 101 Life Safety Code. The assessment will encompass the exterior grounds as well. The contractor will consult with the facility's Engineering and Safety point of contacts (POC) to identify areas of the facility that meet the mixed occupancy criteria outlined in NFPA 101. e.Contractor will update Life Safety Drawing for building surveyed. Drawing updates will include verification of smoke zones, fire wall separations for each rating, shafts, suites and exits. In addition, contractor shall focus on updating and including suites when applicable to take Advantage of lesser stringent suite requirements. Layering and drawing shall be as directed by Chief Engineer/Facility Manager. 6.DELIVERABLES The contractor is responsible for producing 3 hard sets and one CD of the following: a. For each facility i.Executive Summary ii.Part 2 of SOC iii.Analysis and Report of Compliance "This includes a listing of identified LSC deficiencies "Each deficiency identified must be identified with the appropriate code By chapter, Version, Section and Verse (for example: NFPA 101 - 2012, Ch. 19.1.2 iv.Recommendation for correcting identified deficiencies v.Cost estimate for correcting identified deficiencies "For each identified deficiency by building, floor and discipline vi.LSC Assessment Matrix that cross-walks the LSC with the Joint Commission Standards developed in Microsoft Excel (latest version) b.The Life Safety Assessment and the Analysis and Report of Compliance will include and review the following at a minimum: i.Fire barrier integrity ii.Fire door rating and/or performance (closes, latches, edge tolerances) iii.Sprinkler spacing in areas which are sprinklered iv.Fire alarm device placement v.Life safety code compliance vi.Location of fire and smoke dampers and review of testing records vii.Sprinkler zone boundary conformance with smoke compartment boundaries (a VA Fire Protection Design Manual requirement) viii.Fire drills and/or emergency evacuation training/drills c.Updated Life Safety Drawings - 3 hard copies and 1 CD to the facility of the LSC drawings developed in Auto CAD (latest version) with PDF version provided in the Analysis and Report of compliance including identification of the following: i.Fire and Smoke barriers and their ratings ii.Fire and Smoke barrier doors and their ratings iii.Occupancy Zones and the LSC they must be in compliance with iv.Egress exit capacities for each stairwell v.All Applicable suites with the square footage of each suite SPECIAL REQUIREMENTS a. Safety Regulations. i.All applicable safety regulations shall be observed during performance of this contract. ii.The VA Industrial Hygienist and/or Safety Officer shall have full authority to see that the Contractor obeys all safety rules and regulations relative to the performance of this contract. b.Security and Access. i.All Contractor personnel must check in with the VA Police upon arrival at the OPC to receive a temporary identification badge. Contractor Personnel must be clearly identified by company uniforms and name tags when on VA premises. c.Hours of Work and Scheduling of Work. i.Unless otherwise specified in Section 5, all work associated with this contract shall be performed during normal working hours (Monday-Friday, 8:00a.m.-4:30 p.m.), unless specifically authorized by the COR. d.Work Site Responsibilities. Contractor shall be responsible for ensuring that work area is clean and free of debris upon completion of work. Any hazardous conditions shall be reported to the COR immediately. Federal Supply Holders, 8(a) Business, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran- Owned Small Business, HUB Zone Small Business, Small Disadvantaged Business, and Women- Owned Small Business concerns may respond by submitting a qualifications letter stating the following information: 1.Capability Statement that exhibits clear and convincing evidence that your firm has the Capabilities and experiences to perform the listed tasks or deliver the required items as stated in the above Statement of Work. 2.Name 3.Company Address 4.Point of Contact 5.Phone Number and email address 6.DUNS # 7.Business Size Determination 8.Qualifying Small Business status (i.e., SDVOSB, VOSB, WOSB, ETC ¦) 9.Certification of small business status The Department of Veteran Affairs, NCA 8 NF/SG is seeking responsible sources to provide service as outlined in the Statement of Work.
