Description

THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The NCO 19, Rocky Mountain Acquisition Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a single or multiple award contracts. The Government will use responses to this notice to make to an appropriate acquisition decision. To furnish Grand Junction VAMC Laboratory with Cadasil testing in accordance with all terms and conditions, provisions and the schedule of the solicitation with the Department of Veterans Affairs Medical Center in Grand Junction, 2121 North Avenue, Grand Junction, CO 81501. The quantity of tests ordered will depend on the VA Medical Center Laboratory Providers' orders. Cadasil Testing will be performed at the Contractor's laboratory. Cadasil testing is CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) is the most common form of hereditary stroke disorder, and is thought to be caused by mutations of the Notch 3 gene on Chromosome 19. The contractor will be responsible for the quality of the result by performing the proper and necessary calibration, running control samples, and properly maintaining the instruments. The contractor will provide an itemized bill to the laboratory within 10 days after the end of the month. This requirement will be given on a cost per test basis. The anticipated period of performance will be one basic year plus four (4) one-year pre-priced options for renewal. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist Juanita Street at Juanita.Street@va.gov no later than 12:30 p.m. (Mountain Time), March 7, 2013. Acquisition Plan VA259-13-AP-0895-Genetic Cadasil Testing has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 62151 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of $7.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract.
