- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 13 Simplified Acquisition Procedures and FAR 12.6 Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as a request for quotation (RFQ). Provisions and clauses in effect through Federal Acquisition Circular 05-65 are incorporated. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acqnet.gov/far and www.acq.osd.mil/dpap/dfars/index.htm. The NAICS code is 332911, with a small business size of 500 employees. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a proposal. The Naval Medical Logistics Command request responses from qualified sources capable of providing Hydrant Backflow Prevention Devices with the following Essential Characteristics. ESSENTIAL CHARACTERISTICS: The Hydrant Backflow Prevention Devices shall consist of: 1.0Scope: This Statement of Work (SOW) details specific requirements for a contract supported procurement of 61 Hydrant Backflow Prevention Devices for Navy Medicine ™s Chemical, Biological, Radiological, and Nuclear Decontamination efforts at 45 Military Treatment Facilities (MTF ™s) throughout the world. 2.0Requirements: The contractor will be required to assemble and ship 61 Hydrant Backflow Prevention Devices to 45 MTF ™s consisting of the following: Brass Hydrant Backflow Prevention Devices without dump, with female 2 ½ inch NST swivel connectors input, a 2 1/2" NST output, and ball valves; each device will include a support Bi-pole (compact bipod, 3 section legs which extend from 16 inches to 38 inches). 3.0Shipping and Packing: Each Backflow Prevention Device will be shipped to prevent damage and meet all shipping standards and requirements. The following are the amount of devices and addresses of locations where the item will be shipped. Delivery locations are provided as an attachment. Other considerations: 1. All equipment and accessories shall be new from the manufacturer. Used or refurbished equipment, or equipment obtained from a third party shall not be accepted. 2. Cost of shipping equipment overseas, if applicable, needs to be considered. Regulatory Requirements: The Hydrant Backflow Prevention Devices and all of the components, shall be compliant with the Food and Drug Administration (FDA) requirements to market and deliver medical products for use in the United States of America, even should delivery be requested outside of the United States. The claims made for the product shall comply with the regulations of the FDA with respect to products for marketing and delivery of a medical product for use in the United States of America, even should delivery be requested outside of the United States.
