This is a sources sought announcement only and is not a request for proposals or quotes. The Government does not intend to make an award on the basis of this Sources Sought Notice or otherwise pay for the information solicited herein. The purpose of this notice is to identify potential sources and does not commit the Government to make an award and /or issue a solicitation. The Atlanta VA Medical Center (AVAMC), 1670 Clairmont Avenue, Atlanta GA.30329 is seeking qualified vendors to provide a Colonoscopy Services to the beneficiaries of AVAMC in accordance with the following specifications: The intent of the contract is to provide timely completion of colonoscopy services, specifically CPT (Current Procedural Terminology) codes 45378, 45380, 45382, 45384 and 45385. Agencies are to be located within 25 miles of Atlanta VAMC hospital. The applicable NAICS code is 621399 -Miscellaneous Healthcare This notice is NOT A REQUEST FOR COMPETITIVE PROPOSALS. However, interested parties may identify their experience in providing these services by submitting the following information: (1) STATEMENT OF CABABILITY: Submit a brief description [three (3) pages or less] that shows your company can provide the services. Include past experience in providing the same to the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your ability to provide coverage as identified above. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS relative to NAICS 621399 (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation can only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.
