- The Arkansas Army National Guard, USP&FO Office intends to issue a Blanket Purchase Agreement (BPA) for physical examination services at Camp Joseph T. Robinson, AR and Fort Chaffee, AR. The acquisitions is restricted to Small Business's who are listed under NAICS code 621111, Offices of Physicians, size standard $10,000,000. less than br / greater than 2 BPAs will be issued, one for Camp Robinson and one for Fort Chaffee examination requirements. Solicitation will be issued and awarded using the simplified acquisition procedures of FAR 13.303. The BPA will be issued to the offeror who has the overall lowest prices for examination services and has an examination facility within 10 to 15 minutes drive of the main entrances to Camp Robinson or Fort Chaffee. The clause 52.204-99 -- System for Award (SAM) Management Registration is applicable to this requirement. Contractors must have an active registration and record in SAM https://www.sam.gov. Contractors who are not registered in SAM as of the date of submission of a quotations will not be considered for award. less than br / greater than less than br / greater than Exams will be conducted on individuals employed in certain job positions requiring pre-employment, annual/baseline, and end of employment physical examinations. Physical examinations will be conducted by Physician(s) credentialed in the state of Arkansas, who have privileges at their clinic site, and whose licenses do not have practice restrictions. less than br / greater than less than br / greater than Services must be performed during Camp Robinson's normal duty hours of 7:00 A. M. to 3:30 P.M., Monday-Friday. Appointments will be coordinated by the Arkansas Army National Guard Occupational Health Office personnel. The number of days that physical examinations are to be conducted can be no less than four (4) days in the Monday-Friday work week, due to the need to place personnel who are hired for pre-placement in a job in a timely manner. The total number of annual examination for Camp Robinson is estimated at 200 each and 50 each for Fort Chaffee. Calls will be made on a as needed basis.
