- The National Cemetery Administration is soliciting offers under solicitation number VA786-13-R-0048 for the following cemetery: South Florida National Cemetery 6501 S. State Road 7 Lake Worth, FL 33449 PH: (561) 649-6489 This acquisition is 100% set aside for Small Businesses. The NAICS Code is 221310, Size Standard is $7.0 Million. The Contractor gaining award shall furnish all labor, tools, materials, parts, equipment and testing to Provide Preventative Maintenance Service on the Potable Water Treatment System at South Florida National Cemetery, Lake Worth, FL. GENERAL REQUIREMENTS 1.Provide Preventative Maintenance Service on Potable Water Treatment System to ensure all required performance, operation and compliance standards are maintained. a.Provide a Class "C" Certified Operator licensed in the state of Florida to service, operate and maintain a category three (III) water treatment plant. b.Make necessary adjustments to equipment and perform testing to maintain required chlorine dosing level (residual), system pressures and overall performance requirements per applicable permits, laws and regulations. The operator shall test chlorine residual at most distant location in the water system during every site visit and record results in the maintenance logs. c.Preventative maintenance shall be conducted in strict compliance with Chapter 62-555.305 Florida Administrative Code and the water treatment system Operation & Maintenance Manual. (1) One copy of the plant's manufacturer data and O&M manual will be provided at South Florida VA National Cemetery for the contractor's reference. The manual shall remain Government property and shall not be removed from the site without written authorization from the Contracting Officer's Representative (COR). The contractor may make copies of the manual at the contractor's expense. d.Provide and deliver all required water treatment chemicals to the site and check/fill the chemical feed tanks to appropriate operation levels. All chemicals used on this system shall be NSF certified. e.If the system must be temporarily shutdown to complete preventative maintenance service, the contractor shall provide the COR a minimum 24-hour notice. Upon completion of maintenance service, the operator shall immediately return the system to full operation then notify the COR that the system is available for use. f.The operator shall make (1) one site visit each month to perform preventative maintenance. 2.Offerors shall have a current State of Florida, Class "3C" water treatment plant certified operator license PRIOR to submission of bid. The Contractor shall provide to the contracting officer copies of the following with submission of bid/proposal: a.Certified Operator: Name, State of Florida Certified Operator's License(s), Class "C", Category III Water Treatment Plant (for each proposed operator). b.Laboratory: Department of Health Laboratory Certification to perform drinking water analyses. c.NSF Certification for all water treatment chemicals to be used Additional requirements are described in the Statement of Work that will be contained in the solicitation when it is issued. All work shall be performed in strict accordance with the terms and conditions of the contract for the base period of Date of Contract Award through September 30, 2013 with four (4) one year options to extend the term of the contract, if exercised. Evaluation criteria for this award will be in the following order: 1) Price, 2) Past Performance, 3) Technical. Technical and past performance, when combined, are approximately equal to cost or price. Electronic solicitations will be available for download from http://www.fbo.gov on or about December 28, 2012. Proposals are due at 2:00 P.M. on January 11, 2013. Any amendments to the solicitation will also be available at www.fbo.gov. Offerors are advised they are responsible for obtaining any amendments.
