- This is a posting for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442723330692. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-63, Effective 10 Dec 2012, DPN 20121212 effective 12 Dec 2012, and AFAC 2012-1107, effective 07 Nov 2012. This acquisition is a sole source requirement. The North American Industry Classification System (NAICS) code is 339113. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 5047. The proposed Sole Source is US BIOSERVICES CORP, 3200 INTERNET BLVD, Frisco TX, 75034-1950. A Sole Source letter to US BIOSERVICES CORP has been attached to this solicitation. Travis AFB intends to award a purchase order for a Carticel Cellular Implantation (lab cultured cartilage). CLIN0001: Requirement Description: Carticel is an analogous cellular product for the repair of symptomatic cartilage defects of the femoral condyle, caused by acute or repetitive trauma. Qty = 1 EA Please provide the unit cost for this item. Include the cost of shipping FOB destination in your quote. Award shall be made in the aggregate, all or none.
