- Questions & Answers Modification to extend RFQ submission date and revised SOW. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is available on the FBO website. RFQ number W912PQ-13-T-0005 is being issued as a Request for Quote. This procurement is unrestricted. The cost magnitude is estimated between $750,000 and $2,000,000.00 per year. This is a Base year plus 4 Option Years Contract. The periods of performance are as follows: 1 Jan 2013 - 31 Dec 2013, Base Year 1 Jan 2014 - 31 Dec 2014 Option Year 1 1 Jan 2015 - 31 Dec 2015 Option Year 2 1 Jan 2016 - 31 Dec 2016 Option Year 3 1 Jan 2017 - 31 Dec 2017 Option Year 4 Mobile Dental Services are to be provided in support of Individual Medical Readiness Including Exams, Treatment and DENCLASS Entry. Contractor shall provide management, labor, material, equipment, certifications and supplies necessary to provide all listed services utilizing a HIPAA Privacy Act, OSHA and ADA Compliant mobile health facility or protable equipment in accordance with the Statement of Work which is attached to amd made part of this solicitation.
