- Description
- 1. The contractor shall provide Cardiothoracic Physician Assistant(s) (PA) to perform services at the Clement J. Zablocki VA Medical Center, 5000 West National Avenue, Milwaukee, Wisconsin. The VA Chief of the Cardiothoracic Surgery Section will provide technical and professional direction. Other members of the Cardiothoracic Surgery Section attending staff are designated alternates. 2. The following duties and skills are required, but are not limited to: a. Assist in the operating room as the first or second assistant on a daily basis. As required, assist in the day-to-day management of patients on the surgical wards and in the ICU. This includes writing admission and discharge orders, transfer orders from ICU to other wards, medication orders, IV orders, and all other orders that are potentially required for the management of patients. A resident or staff physician will cosign these orders within 24 hours. The PA is allowed to dictate discharge summaries, which must be countersigned by the attending staff. b. Initiate consultations on inpatients or outpatients clinics as directed by the CT Surgery Staff or resident physicians. c. Conduct and report results of CT surgery testing and other tests or procedures when indicated and approved by the supervising physician. d. Certain routine duties may be performed by the PA as part of the routine work assignments without specific discussion with the supervising physician. These duties are always regulated by the Approved Clinic Privileges of the individual. They include inject IV medications; start blood transfusions or intravenous infusions of any variety, as long as hospital protocols regarding preparing of skin sites; checking of blood products and attention to sterile technique are observed; and change dressing on surgical wounds. e. Perform the following tasks only under the direct supervision of a staff or resident physician who is physically present at the time of the procedures: place central lines; place chest tubes; and place pleureX catheters. f. Assist in the OR as a surgical assistant to help open and close all incisions pertinent to CT surgery. Act as first or second assistant to the resident or to the staff physician in the operation room. g. In an emergency situation, pending arrival of a physician, the PA may perform emergency care within the scope of Clinical Privileges and according to the VA Medical Center policies and procedures. The PA may perform any procedure; administer or order any medications that would normally be ordered; order any emergency resuscitative drugs required during an attempted Code procedure, until a senior staff physician or resident physician is present; and perform CPR and assist in any capacity, including mechanical resuscitation or surgical operative resuscitation, required by the physician at hand. h. Perform histories and physical examinations on cardiothoracic inpatients and outpatients. The supervising physician will review and countersign all admitting inpatient history and physical examinations. i. Prescribe medications within defined scope of practice. j. Order diagnostic studies such as laboratory tests, x-rays, or electrocardiograms. k. Carry out health promotion and disease prevention activities. l. Obtain venous or arterial blood specimens as needed. m. Make daily rounds to observe and record progress notes in the patient's medical record. Services may be required seven days per week, including holidays. n. Notifiy the responsible physician of significant changes in the patient's medical condition.. o. Provide education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs. p. Harvest vein, close the incision, and assist the surgeon as directed in the operating room. q. Perform routine wound care and debridement as needed. r. Remove chest tubes as directed by the supervisory physician. s. Order CT/MRI or other non-invasive studies. t. Order invasive studies as directed by the supervising physician. u. Maintain quality assurance programs referable to Cardiothoracic Services. Ensure Cardiothoracic services comply with all applicable Joint Commission (JC) guidelines with respect to surgical procedures. v. Comply with appropriate and applicable policies, procedures and regulations of the Milwaukee VAMC. Examples include rules and regulations, narcotic control and use, personnel policies, infection control, and medical records. 3. The Cardiothoracic PA is authorized to perform the following duties as directed. During the performance of the following duties, a staff physician must be available for consultation by telephone, and if summoned, be physically present at the site of the procedure within a reasonable period of time. a. Insert arterial lines. b. Perform thoracentesis under direct supervision of physician. c. Change arterial and central venous lines over a wire. 4. The Cardiothoracic PA is authorized to perform the following duties for patients with a life-threatening situation where a physician is not immediately available. A physician will be summoned as soon as possible. a. Basic Life Support (BLS) in accordance with current Cardio-Pulmonary Resuscitation standards. b. Advance Cardiac Life Support (ACLS) in accordance with ACLS protocols, to include defibrillation and cardioversion if the PA has current documentation of successful ACLS course completion. 5. Patient treatment documentation to any/all clinical records will occur immediately following the procedure and may include documenting in the electronic patient record. VA uses an electronic patient record system and therefore, electronic signatures will be required. 6. The PA may be assigned administrative duties including quality assurance, peer review, serving on various committees as directed by the Program Director or administration, and as deemed appropriate by the managing physician. Other non-clinical tasks may also be assigned. 7. The PA shall be physically present in the facility to perform the required services as specified in this contract. All patient encounters are entered into the VA medical center DHCP/VISTA computer system and shall provide documentation of workload. 8. Hours of Service a. The contractor shall be responsible for providing on call coverage 24 hours a day, 365 days per year. On-call means on a pager and within 30 minutes of reporting at the VA Medical Center. On-call services would include federal holidays. While on-call at the Milwaukee VAMC, the PA's covered by this contract are not to be concurrently on-call to both the VA and another institution for the same periods of time. Alternatively, the contractor will be allowed to create a call schedule whereby there is a primary PA on call with a second PA on secondary call, available for the VA thus always having someone available for call. b. Cardiac surgeries are performed and scheduled Monday through Friday from 6:00am to 6:30pm. Cardiothoracic PA's shall be available for performing services during the scheduled surgeries. The VA Section Chief shall provide in writing a defined schedule of PA availability per days of the week in writing one week in advance. c. The 10 holidays observed by the Federal Government are: New Year's Day (January 1st), Martin Luther King's Birthday (3rd Monday in January), Presidents' Day (3rd Monday in February), Memorial Day (last Monday in May), Independence Day (July 4th), Labor Day (1st Monday in September), Columbus Day (2nd Monday in October), Veterans Day (November 11), Thanksgiving Day (4th Thursday in November), Christmas Day (December 25th) and any other day specifically declared by the President of the United States to be a national holiday. When one of the above designated legal holidays falls on a Sunday, the following Monday will be observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday by U.S. Government agencies. d. Absences: The contractor shall give 30 days written notice to the VA Section Chief when scheduled PA absences are planned. The VA Section Chief must approve any deviation from this requirement. The PA shall inform the VA Section Chief as soon as possible if he/she must leave the clinic during regular duty hours due to illness, emergency or personal reasons. The payment for any leave, including sick leave, vacation time, military leave and educational leave are the responsibility of the contractor.
