Description: PROVIDE ALL EQUIPMENT, WARRANTY, TRAINING INSTALLATION, AND LICENSE FOR THE VISUAL FIELD HFA 750i FOR THE VETERANS HEALTHCARE SYSTEM OF THE OZARKS (VHSO) LOCATED IN FAYETTEVILLE, AR. (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All Small Businesses submitting an offer must be registered on www.sam.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA256-13-Q-0219. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-61. (IV) This procurement is restricted to Small Business Concerns. The associated NAICS code is 423460. The small business size standard is 100 employees. (V) This combined solicitation/synopsis is for purchase, on the following commercial: Item # 0001 HFA 750i with table including USB, DICOM GATEWAY 2.0, DB 2.0 LICENSE, NETPRO, GPA, SITTA SWAP AND KINETIC. Product number 266002-1140-757, or equal QUANITY 1 EACH $___________ (VI) Description of requirements is listed below. Equipment must have the following features/capabilities: "New equipment ONLY; NO remanufactured or "gray market" items. All items must be covered under the manufacturer's warranty. "Must be compatible with Humphrey fields for progression analysis "Equal to the standards for glaucoma care of brand Humphrey visual fields "Compatible with the Medical Centers VISTA imaging system "Provide installation, 1 year warranty, and training within cost of item (VII) Place of delivery is Veterans Healthcare System of the Ozarks, VA Medical Center, Warehouse, Building #8, 1100 N. College Avenue, Fayetteville, Arkansas 72703. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Feb, 2012), applies to this acquisition. (IX) Award will be made to the offeror whose offer, meeting the requirement of this solicitation, is most advantageous to the Government. (X) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2012), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Feb, 2012), applies to this acquisition.
