- This is a notice of intent to award Sole Source Purchase order and is Not a request for a competive proposal under the authority of FAR 6..302-1 Only one responsible no other supplier or Services will satisfy agency rquirements 41.U.S.C. 253 (c) (1). The government intends to solcit a quote and enter into a firm fix price for a Base and four option years with UHS Surgical Services. Additional in servicing can also be scheduled as needed. Statement of Work Laser Services for Department of Perioperative Medicine (DPM) The National Institutes of Health, Department of Perioperative Medicine, Clinical Center is located in Bethesda, MD. DPM provides operating rooms, post anesthesia care, and technical support for approximately 2000 operative cases per year in a research environment. DPM is requesting laser services with laser probes, materials and technical assistance for use at the Clinical Center hospital for the NIH. A certified manufacturer technician for the following lasers: Greenlight XPS, Greenlight HPS, Holmium, C02 Surgical, C02 Clinicon and Nd: YAG (SLT) that incumbent the followings: • One technical training course to all interested surgeons from the manufacturer for use of the laser equipment at no charge to DPM or surgeons. • A trained technician approved by the manufacturer of the laser equipment to assist surgeons in the use of the laser equipment. • The laser technician shall be responsible for setting-up; checking and maintaining the equipment each day while on premises as well as removal and/or storage of the equipment at the end of each day. • Vendor shall be responsible for all transportation of its laser equipment. • Vendor shall use its best efforts to ensure that the laser equipment is current technology, that it is in good working order, and that it has an ample supply of laser probes on hand at all times. • Vendor shall provide a record log of scheduled maintenance and biomedical approvals each time the service is requested. • DPM will pay for the use of additional tips and fibers. We agree to pay for each tip over one (1) that is used per single case. For each urology case in which the 200-micron fiber is used, an additional will be added per case. For each case in which an additional Nd: Yag fiber is used an additional will be added per case. • DPM agrees to pay the vendor fee for each case in which a laser was scheduled and not used during the surgical procedures, after 3:30 p.m. on weekdays, between 3:30 p.m. Friday, and 7:00 a.m. Monday, and major holidays. • DPM desires the vendor to be ready to perform services with requested equipment, supplies, and technical support within a 48 hours notice. • Vendor must maintain appropriate insurance coverage during the term of this agreement. • Vendor agrees to provide proof of certifications and competency of laser tech when requested. The initial period of perform will be for 12 months from the award date of the purchase order. The government has the unilateral right to extend the terms of services for an additional 4 years period. FAR 52.217-9 is hereby incorporate within this order. If the options are excised pricing would be from the original quote that outlined the prices for the next 5 years. Variable surgical caseload precludes the NIH from establishing a set schedule. The Government will pay for the services once used on a case-by-case basis. NO CALLS PLEASE
