- Clauses This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2013-54183 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information: CLIN 0001: $ Each Total Price Equipos Mechanical Support Arm P/N YNA0138260 QTY 16 CLIN 0002 Total Price Equipos Winch, Standard W/Z-AXIX P/N 500-5520-02 QTY 1 This acquisition is under North American Industry Classification System Code 441310. Is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://acquisition.gov.far This product will be evaluated as an occupational safety intervention to prevent shoulder injuries in automotive manufacturing. This study is being conducted in collaboration with Toyota Motors and, through pilot testing, Toyota has identified the Equipois zeroG-2 mechanical arm as the most appropriate workplace control to support nut running torque tools in the assembly of the vehicle chassis. Toyota has conducted preliminary evaluations to determine that this particular device has the highest probability of success as an intervention to prevent shoulder injuries. As such, the evaluation of this specific workplace intervention was proposed in a research proposal that was subjected to a scientific peer review and ultimately awarded funding based on its scientific merits, likelihood of success, and potential for impact. Substitution of another product would jeopardize the probability of success of the project that was justified in the research proposal. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, a contractor MUST be registered in the System for Award Management (SAM). www.sam.gov The quote format is at the discretion of the offeror. Include FAR 52.212-3, Offer Representation and Certification with the offer or indicate it is available on the ORCA website.
