Description
Request For Proposal Document This acquisition is set-aside for small business participation. The Small Business Size Standard is $14 Million. If interested in this project, the company must download the solciitation document using the link to "RFQ 13-161-SOL-00002" shown in the window on the right side of this webpage. Project Summary: The Gallup Office Building 2016, so-called Building F, is currently experiencing various structural problems associated with the interior perimeter wall resulting in cracking and minor buckling of interior walls along the north side, first floor. The two story unit includes a basement was initially constructed in 1965 encompassing approximately 5,682 square feet and is currently utilized as office space. Scope of Work : The Selected contractor shall provide the necessary manpower, equipment and services for the following services: 1. Visit the project site and meet with Indian Health Service (IHS) staff personnel and inspect the facility structural problems. If any geotechnical investigations or special testing is required, the engineer shall identify and itemize as an optional cost for consideration by the government. 2. Once the onsite structural investigation has been accomplished, including other testing and inspections, the engineer shall prepare a report addressing findings, conclusions, recommendations to repair (with alternatives), cost estimates and schedule to repair. The report shall also provide a plan of action to accomplish the recommendations. 3. Optional Item : Development of Construction Documents that identify and outline the Scope of Work for the recommended repairs including construction administration services, onsite periodical onsite inspections, submittal reviews, and final as-built documents and final inspection. Deliverables and Time Limitations : Within 30 days after receipt of the award, the selected contractor shall inspect the facility and provide the IHS for review and acceptance, a detail inspection report identifying the cause of the structural problems including recommendations on corrective measures to resolve the foundation problems that currently exist at the facility.
