W-9 VENDOR ACKNOWLEDGEMENT FORM TERMS AND CONDITIONS FAR CLAUSES UNIFORM RFQ ETR/Iroquois Job Corps Center 11780 Tibbets Rd., Medina NY 14103 Subject: Request for Quotation (RFQ) - Uniforms and Clothing Solicitation # Bid 2013-39IJCC Uniforms 2013 Thank you for expressing interest in providing uniforms and clothing for the Iroquois Job Corps Center. Products to be provided are outlined in the Scope of Work. All bids must be returned in a sealed envelope or emailed and received no later than 4:00 PM on December 10, 2012. This RFQ does not commit ETR to pay any costs incurred in the preparation of a cost proposal or quotation, nor does it commit ETR to award a contract for products, supplies, or services. ETR will be the sole judge as to which bid offers the best overall value and will serve the best interest of ETR. ETR reserves the right to reject any and all bids without recourse from bidders. Washable 65/35 blend material that requires little to no ironing. approximately 1000 pairs of pants ordered per year. Women Khaki front pleated Pants. Size range from 0- 26-28 (larger sizes if available). Approximately 800 pairs of pants per year. Polo shirts: Color to be maroon or burgundy only. Pique knit in men's sizes small - 6 xlg.
