Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 and supplemented with additional information included in this notice. The Department of Veterans Affairs 646 is considering whether or not to set aside an acquisition for Small Business. The solicitation number is VA244-13-Q-0130. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The NAICS Code is 541990. Successful offer will be the one that provides the Lowest Price Technically Acceptable. The award will be for 12 months of service (Base year) and four option years. The VA Pittsburgh Healthcare System 646 has a requirement for the services listed below. Perform quarterly trace gas testing monitoring and documentation by report of the waste anesthetic gas machines used at the VA Pittsburgh Healthcare System, University Drive C., Pittsburgh PA. 15240. The frequency of monitoring for this service will be for quarter 1 (completed no later than December 15, 2012), quarter2 (Completed no later than March 15,2013), quarter 3 (Completed no later than June 15, 2013), and quarter 4 (Completed no later than September 15, 2013). Testing will include: Operating/Recovery rooms, (OD) 1 through 10, ASU Room 3, the EP lab, MRI machine, and the portable anesthesia machine. All machine leaks which exceed the NIOSH maximum limit of 25 ppm for nitrous oxide must be reported to the Safety Department, Supervisor of Biomedical Shop, or designated hospital representative prior to the company representative leaving the medical center. The vendor is authorized to make minor reasonable repairs within the scope of testing/preventative maintenance as certified repair technician of the subject equipment. Vendor will request per approval from Contract Office for critical immediate repair at contract hourly rate. All communications regarding quotes, questions, etc. shall be submitted by email to John Barton (Johnny.Barton@va.gov). Phone calls will not be accepted. Quotes are due by 12/4/2012 at 9am Eastern Standard Time. Quotes shall be submitted by email.
