Services to be provided are outlined in the Scope of Work. This RFQ does not commit ETR to pay any costs incurred in the preparation of a cost proposal or quotation, nor does it commit ETR to award a contract for products, supplies, or services. ETR will be the sole judge as to which bid offers the best overall value and will serve the best interest of ETR. ETR reserves the right to reject any and all bids without recourse from bidders. Medina, N.Y. 14103 Office: (585) 798-7024 Tina.burrage@etrky.com RESPONSE TO SOLICITATION To be considered for an award of the contract, the response to this solicitation will include: Completed "Bid Sheet" with specific Fee-For-Services information Subcontractor will provide all applicable licenses in accordance with New York State & Federal Regulations for all employees and corporate associates to render services on center All applicable certificates of Insurance Proposed schedule (days/hours) of specific services to be rendered A vendor acknowledgement and W-9 The most suitable candidate may be interviewed prior to the award of the subcontract. BID PROVISIONS The following Bid Provisions will apply. Failure to carefully read and follow all instructions will not relieve the bidder of any bid provision, and failure to do so will not place ETR at risk. All prices and notations must be in ink or typewritten. No erasures will be permitted. Errors may be crossed out and corrections printed in ink or typed adjacent and must be initialed in ink by individual signing the bid. Price Warranty: The Fee-For Services quoted herein are no higher than would be quoted to any other customer, be they commercial and/or agencies or the United States Government for similar services. The bidder will list on a separate sheet of paper any expectations to the conditions of this bid. This sheet will be labeled "Expectations to Bid Conditions" and will be attached to the Bid Sheet. If no exceptions are stated, it will be understood that all general and specific conditions will be complied with without exception. ETR reserves the right to accept and/or reject any or all bids or waive any formality connected with the bids. Lowest rate or overall bid does not constitute a Bid Award. ETR will be the sole judge of which bid is selected for approval (see "Selection of Contractor" above) and subcontract award. Items will be quoted F.O.B. destination. No additional charge for delivery, drayage, express, parcel post, packing, cartage, insurance, license fees, permits, cost of bonds, or for any other purpose will be paid by ETR unless specifically included in the bid and accepted by ETR. In the event that subcontract is awarded in response to this RFQ, the below Statement of Work, provisions and set forth Terms & Conditions will be incorporated in their entirety into the subcontract and become an integral part of the subcontract when the subcontract is accepted by the subcontractor, either by acknowledgement or commencement of performance. Iroquois Job Corps Center 11780 Tibbets Rd. Medina, NY 14103 CENTER FIRE SAFETY SYSTEMS SERVICES SCOPE of WORK The center fire safety systems services subcontractor, referred to henceforth as the Subcontractor, agrees to provide the Iroquois Job Corps Center, referred to henceforth as the center, with the services stated herein. Services will be provided as authorized by the IJCC Administrative Services Director or his/her representative. Remuneration for services rendered will be at the fee-for-services price. The period of performance for this subcontract will be from 1 February 20113 through 31 January 2014. Services will be provided on a previously scheduled basis. Successful vendor will provide services as detailed herein and according to the previously agreed upon schedule of services provided by the center as follows: •A. Subcontractor will provide Portable Fire Extinguishers Services: Contractor must be able to certify, re-certify, charge, re-charge, tag, re-tag, properly bracket, deploy / re-deploy - (on a rotating and as needed basis), approximately 162 field units per Federal, New York State and local fire prevention codes. Vendor must provide a computer program which ensures that all services, test results, mitigation activities, and certifications, are fully addressable via remote terminal with center safety officer email notifications of same in real time - (subcontractor will provide all materials and labor necessary to fulfill this section). Subcontractor will provide Fire Alarm Detection Systems Services: Contractor to provide quarterly inspections, cleaning, testing, and replacement (as needed) of Fire Alarm Detections Systems field devices: Contractor must ensure that all services, test results and certifications must be able to be entered into a computer program which must be fully addressable via remote terminal with tracking up to the minute, center safety officer automatic notification of serious issues via email and mitigation techniques, as well as computer archived information services. Subcontractor will provide Fire Suppression Systems Services: (1 Fire Pump, 3 Kitchen Hood Systems, 3 Bldgs. with sprinkler systems) Contractor will provide service, testing, certification, re-certification, maintenance, repair, and redeployment ordering activities as needed, as well as ensuring system compliance with Federal, New York State, and local fire prevention codes. All services, test results and certifications must be able to be entered into a computer program which must be fully addressable via remote terminal, tracked, and archived electronically with notification of safety issues and mitigation activities sent to center safety officer automatically via email. •D. Subcontractor will provide Emergency & Fire Exit Lighting Units Services: Contractor will provide monthly testing, service, battery replacement, bulb replacement and replacements of unit that are no longer operational (as needed) of emergency lighting and exit lighting units. All service activities, test results and certifications must be able to be entered into a computer program which must be fully addressable via remote terminal, with center safety officer notification of mitigation techniques used to render acceptable outcomes - must be archived electronically - (subcontractor will provide all materials and labor necessary to fulfill this section). This Scope of Work is intended to be an all inclusive & complete Fire Safety Systems Program Services agreement and as such should act in accordance with all the most recent and most stringent regulatory compliances offered between local, state, and federal auspices. Subcontractor will provide identification of corporate associates who will render services on center, proof of those individuals motor vehicle licensure, and ensure effective coordination with the center Logistics Manager in rendering all on center services per the center's sign in/sign out procedures Subcontractor agrees to comply with any and all safety and security processes and protocols throughout the duration of the performance period Subcontractor will ensure that all corporate associates will not fraternize with center students or staff while on center in the performance of professional services Vendor will bear the full cost, and replacement costs of all items related to the fire safety systems and field devices mentioned herein. Vendor will provide a detailed monthly report to the center, identifying all deficiencies in any and all of the fire safety systems. This report is to also identify the mitigation process of all deficiencies- including a time table to correct these deficiencies.
