- CABLE AND INTERNET SERVICES, ALEDA E LUTZ VAMC, 1500 WEISS STREET, SAGINAW, MI 48604 (See attachment for Work Statement and Price Schedule): This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-61B. The North American Industry Classification System Code is 517110 WIRED TELECOMMUNICATIONS CARRIERS, INTERNET AND CABLE TELEVISION. The small business size standard is 1,500 employees. This procurement is 100% set aside for Veteran Owned Small Business (VOSB). Offerors are encouraged to submit quotes based on the work requirements specified herein for a base period from date of award (approximately January 1, 2013) through December 31 2013, with FOUR pre-priced option years from January 1 through December 31. A site visit is planned for November 29th at 8:30am, Aleda E. Lutz VAMC, Building 1, conference Room 5A, 1500 Weiss Street, Saginaw, MI. Proposed rates should include all costs associated with Request For Proposal VA251-13-R-0043 as follows: Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (APR 2012), with its offer. The clause at 52.212-4, Contract Terms and Conditions - Commercial Items (FEB 2012), applies to this acquisition with the following clauses added as addenda to the clause: 52.216-18 Ordering, 52.217-8 Option to Extend Services, 52-212-9 Option to Extend the Term of the Contract (MAR 2000), 52.224-1, Privacy Act Notification, 52.224-2 Privacy Act (APR 1984), VAAR 852.203-70 (Commercial Advertising (JAN 2008), 852.2037-71 Display of Department of Veteran Affairs Hotline Poster (DEC 1992), and VAAR 852.273-76 Electronic Invoice Submission (Interim -October 2008). The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items (AUGUST 2012), applies to this acquisition and the additional FAR clauses cited in the clause are: Paragraph (b) (1), (4), (5), (10), (12), (14), (23), (26), (28), (29), (30), (31 ), (32), (34), (38), (41), (42), and (48) and Paragraph (c) (1), (2), (3). The following provisions are included: 52.21-1 Instructions to Offerors-Commercial Items (FEB 2012), 52.209-7 Information Regarding Responsibility Matters (FEB 2012), 52.216- Type of Contract (APR 1984), 52.219-22 Small Disadvantaged Business Status (OCT 1999), 52.228-5 Insurance-Work on a Government Installation, VAAR 852.233-70 Protest Content/Alternate Dispute Resolution (JAN 2008), VAAR 852.233-71 Alternate Protest Procedure (JAN 1998), 52.212-2 Evaluation-Commercial Items (JAN 1999), VAAR 852.237-7 Contractor Responsibilities. Service Contract Act Wage Determination 2005-2275 Revision 12 applies. Proposals will be evaluated on the basis of lowest priced, technically acceptable. Proposals are due no later than 1:00 p.m., local time, December 17, 2012. Submissions must include a completed price sheet (attached), Representations and Certifications, and proof of liability insurance. Proposal shall also include a brief narrative explaining installation and equipment requirements and timeline from award until becoming fully operational.
