This RFQ does not commit Education & Training Resources (ETR) to pay any costs incurred in the preparation of a quotation, nor does it commit ETR to an award for products, supplies, or services. Professional Qualifications : Subcontractor must be a fully certified/licensed psychiatrist, clinical psychologist or psychiatric social worker. Subcontractor shall be an independent Subcontractor and not an employee or agent of ETR/Iroquois Job Corps Center or the US Dept. of Labor. Subcontractor shall maintain all appropriate licenses, certifications and registrations for the State of New York and liability insurance and shall provide proof thereof to ETR/Iroquois Job Corps Center. Experience in the following applied areas is desirable and may be used in a weighted decision. •· Previous experience in a residential school or treatment facility serving youth, such as a hospital or halfway house. •· Demonstrated understanding of accommodating students with special needs. •· Demonstrated knowledge of community resources as obtained by work in a vocational rehabilitation office or a community center. •· Demonstrated understanding of disadvantaged youth as obtained through prior employment or through personal experiences. •· Demonstrated ability to provide for staff development needs as obtained through previous organizational development. Clinical Responsibilities include: •· Advising the center director and staff of preventive mental health measures. •· Providing early recognition of behaviors requiring clinical advice or referral. •· Consulting on monitoring the mental health referral and feedback system. •· Conducting clinical interviews for purposes of evaluation, diagnosis, and disposition for all new students. •· Performing or arranging psychological testing services in limited situations as needed and authorized. •· Preparing case management plans with a focus on employability that identify specific expectations of staff and the student/employee. •· Maintaining clinical case histories and required communications, records and forms, including provisions for confidentiality. •· Providing clinical support to the TEAP specialist, as needed •· Facilitate weekly group sessions to enhance student experience and improve favorable outcomes. •· Attend twice weekly Outcome Measurement Systems (OMS) meetings and provide input to team regarding issues, solutions, and impact on student success rates. •· Review potential student employee folders to identify special needs and accommodations that may be required for these students. •· Provide staff training on an ongoing basis as well as during summer and winter breaks in order to increase knowledge and understanding of staff with issues and problems faced by Iroquois Student population. •· Work with Wellness Manager and TEAP Specialist to ensure students receive proper support and resources to cope with physical and/or emotional dependencies. •· Attend off campus Celebrate Recovery meetings on a weekly basis to support students with in coping with physical and/or emotional dependencies. RESPONSE TO SOLICITATION To be considered for an award of the subcontract, the response to this solicitation will include: Completed "BID SHEET" with Hourly Rates of service. Bidders must include proposed rate for one base year and two option years as appropriate Valid licenses to practice in the state of New York Valid DEA license, if applicable (see DEA License section under Terms & Conditions) Certificate of Insurance (see Insurance section under Terms & Conditions) Resume or Curriculum Vitae Statement of Experience in dealing with adolescents Proposed schedule (days/hours) Individual Hourly Rate Breakdown form, if applicable SELECTION and EVALUATION CRITERIA OF CONTRACTOR In addition to the hourly rate of service, selection of the subcontractor will be based and evaluated using the following criteria: Experience in providing mental health care for adolescents and young adults (minimum of five (5) years experience) Provision of mental health services to youths who are socially and economically disadvantaged Previous practice and experience in settings such as schools, colleges, institutions, military, or correctional institutions Willingness and ability to carry out required duties and responsibilities Admission and attending privileges at local hospitals or specialty clinics Academic affiliation with nearby professional schools or colleges Approval of the subcontractor by the ETR Corporate Office Approval of the subcontractor by Regional Office of Job Corps, U.S. Department of Labor The most suitable candidate(s) will be interviewed prior to subcontract award BID PROVISIONS The following Bid Provisions will apply. Failure to carefully read and follow all instructions and specifications will not be cause to negatively impact the center and are at the bidder's risk. All prices must be in ink or be typewritten. No erasures are permitted. Errors may be crossed out and corrections printed in ink or typed adjacent and must be initialed in ink by the person signing bid. Price Warranty: The Hourly Rates quoted herein are no higher than would be quoted to any other customer commercial and/or agencies or the United States Government for similar services. The bidder will list on a separate sheet of paper any exceptions to the conditions of this bid. This sheet will be labeled "Exceptions to Bid Conditions" and will be attached to the Bid Sheet. If no exceptions are stated, it will be understood that all general and specific conditions will be complied with without exception. ETR reserves the right to accept and/or reject any or all bids or waive any formality connected with the bids. Lowest rate or overall bid does not constitute bid award. ETR will be the sole judge of which bid is selected for approval (see "Selection of Subcontractor" above) and contract awardee by letter. Items will be quoted F.O.B. Destination. No additional charge for delivery, drayage, express, parcel post, packing, cartage, insurance, license fees, permits, cost of bonds, or any other purpose will be paid by ETR unless specifically included in the bid and accepted by ETR. In the event that a subcontract is awarded in response to the RFQ, the below Statement of Work, provisions and set forth Terms & Conditions will be incorporated in their entirety into the subcontract and become an integral part of the subcontract when and if the subcontract is accepted by the subcontractor, either by acknowledgement or by commencement of performance. Please see attached link for obtaining a DUNS number: http://fedgov.dnb.com/webform Please see attached link for registering with the CCR: https://www.sam.gov/portal/public/SAM/ CENTER MENTAL HEALTH CONSULTANT IROQUOIS JOB CORPS CENTER SCOPE OF WORK The Center Mental Health Consultant, referred to henceforth as the center consultant of Subcontractor, agrees to provide students of the Iroquois Job Corps Center, referred to henceforth as the Center, with the services stated herein. Services will be provided as authorized by the Center Director or his/her representative, who will provide a schedule of hours and days to be worked. Payment for services rendered will be according to the agreed upon hourly rate for (12.75) hours per week, on average, fifty (50) weeks per year. The base period of performance is Feb. 1 st, 2013 Through January 31 st, 2014, with two (2), one year, pre-priced option years. The center and the Center Mental Consultant hereby agree that the services to be provided will include, but will not be limited to, those mental health services specified in the Job Corps Federal Regulations (Title 20 Code of Federal Regulations, Part 670.525) in the Job Corps Policy & Requirements Handbook (PRH) chapter 6, and the applicable Technical Assistance Guides (TAGs), http://jchealth.jobcorps.gov. Responsibilities: Staff development activities are essential to establishing an effective mental Health and Wellness Program. Planning for the provision of these services should be based on student and staff needs. The Center Director should meet regularly with the center Consultant to discuss the status of the program, the staff contribution, and current student problems. The center consultant may serve as principal technical advisor to the Center Director and senior staff on managing organizational development. Organizational analyses may be conducted by examining patterns for delegation of authority and responsibility, observing the planning cycle and implementation of operational plans, analyzing interacting management styles, and assisting in developing needs assessments. These observations, their possible meanings, and various alternative models can be shared with the center management team with a view toward improving organizational effectiveness. Staff development is enhanced when the center consultant works with staff members individually or in groups on "applied" topics. For example, through case conferences, the center consultant and a teacher may work on how to motivate beginning reading students; or the center consultant and residential living staff may discuss potential mental health problems encountered in the dorms. These technical assistance sessions can count toward fulfilling requirement for general skills training: they provide on-the-job staff development. The center consultant contributes to staff training requirements through the staff development modules. These training modules are a basic set of sessions. The basic presentation, outline, overhead text, and handouts are provided for each module. Other modules can be performed by the center consultant, but staff training experiences should not be limited to the center consultant presentations. The center consultant may be asked to participate in orientation sessions for admission counselors by making presentations on the center mental health and wellness program, the role of the center consultant in Job Corps, and mental health-related review concerns. These presentations should be aimed toward helping admission counselors understand their role and the importance of getting complete information. The center consultant responsibilities should include: •· Evaluation of applicant folders and disability accommodations •· Advising the Center Director and staff on preventive mental health measures •· Providing early recognition of behaviors requiring clinical advice or referral •· Consulting on and monitoring the mental health referral and feedback system •· Conducting clinical interviews for purposes of evaluation, diagnosis, and disposition •· In limited situations, performing or arranging psychological testing services as needed and authorized •· Preparing case management plans that identify specific expectations of staff and the student whose adjustment is at issue. These plans should have a focus on employability skills. •· Maintaining clinical case histories and required communications, records, and forms, including provisions for confidentiality •· Providing clinical support to the TEAP specialist, as needed •· Provide coverage during vacations and/or extended illness. B. Staff Development: Advice to the Center Director and health services staff on all mental health matters and overall center program and organizational consultation to the Center Director and Administrative Staff on a regular basis. This will include staffing requirements. Consultation on the development of a system to promote the coordination and integration of the mental health program with other programs and activities such as counseling, residential life, and recreation. This will be done in cooperation with the health services manager, center physician, center dentist, and other health professionals, as well as any staff member involved with students. Advice and assistance to health education instructors on the implementation of the health education program to include mental health and wellness. Consultation with center health staff, counselors, day advisors, instructors, and other appropriate staff concerning the ongoing mental health problems of students. Staff development on using mental health principles and techniques. This will include coping with problems of alcohol and drug use, sexuality, aggressive behavior and poor communications. Assistance with program development for the purpose of meeting the life crisis of students in constructive ways; for example involvement with the center disciplinary program, the maximum benefits program; the intergroup relations program, and TEAP program. Participate in weekly Interdisciplinary Team meetings to facilitate and assist with disability initiative and accommodation process. Disability Program: The center shall implement a disability program to provide individualized and coordinated services to all students with disabilities. At a minimum; this program shall include: •· A Disability Coordinator to oversee the program •· Written Policies and Procedures related to the center review of applicant folders •· Reasonable accommodations and the formation of an interdisciplinary team •· A method to accurately collect and submit all required disability data •· A written plan to develop and maintain partnerships with outside agencies and programs that will assist the center in serving students with disabilities •· A written self assessment describing the current programmatic and architectural accessibility of the center with priorities and next steps C. Clinical Services: The general emphasis of the mental health program must be on prevention, early detection, identification of mental health problems, short-term counseling for manageable conditions, and crisis Intervention (such as for rape, death or other serious loss, or emotional reaction to a positive HIV test). Intake, evaluation, and case management of current students. Consultant will review the referral and health records prior to the student intake interview. After the intake interview, document in medical record: findings, working diagnosis based on DSM-IVTR, barriers to employability, and treatment needs determined. If determined that the student needs assistance, then a written management plan is developed and elements of the plan should be discussed with appropriate staff A written referral/feedback system shall be established and the maintenance, confidentiality, and safeguarding of all necessary mental health records shall be assured. In the event of a mental health emergency, the mental health consultant or the center physician shall conduct a mental health evaluation as soon as possible and, when necessary, refer the student for psychiatric care. If neither the consultant nor physician is available, the student shall be referred immediately to the closest emergency facility. Students who are a danger to themselves or others must be supervised continuously until the disposition of their cases is resolved. Develop a counseling component to the mental health and wellness program focusing on students' individual needs and progress in personal and social development, basic education, and vocational training. These counseling services are to be provided by career counselors, residential living staff, and other appropriate staff with your support and guidance. Career counselors are an extension of the mental health and wellness program. Disability Program: Participate in weekly Interdisciplinary Team meetings to facilitate and assist with disability initiative on center and accommodation process. Support the Disability Coordinators in making sure students with mental health disabilities are accurately reflected in the data system with accommodations, when appropriate. Participate in Regional Office Teleconferences with the Regional Mental Health Consultant who is to be determined and can be reached at to be determined. Students stabilized on psycho tropic medication shall be accommodated if judged not to be a danger to themselves or others by the center mental health consultant and center physician (with advice of a consulting psychiatrist, if appropriate) and approved by the Center Director. Information exchange shall occur through regular case conferences with the mental health consultant and other appropriate staff based on individual student needs. D. TEAP Services: •1. Provide clinical assistance to the TEAP specialist. 2. Participate in the intervention and management process. 3. Provide direct support for difficult cases and/or students with co-occurring disorders. 4. Consult with the TEAP Specialist and recommend medical separation, medical separation with reinstatement rights, or zero tolerance terminations for alcohol/drug related diagnosis. 5. Participate in weekly meetings with TEAP Specialist to discuss cases. BID SHEET Mental Health Consultant OFFER TO: Education & Training Resources (ETR), Wellness Department, Iroquois Job Corps Center, 11780 Tibbets Road, Medina, NY 14103 In compliance with the Bid Provisions, Scope of Work, Terms & Conditions, and other specifications contained in this RFQ, the undersigned hereby proposes to provide Mental Health Consultant Services for the center. Bid Price (Hourly Rate) The Hourly Rates herein are no higher than would be quoted to any other customer, either commercial and/or agencies of the United States Government, for similar services. If bidding as an independent consultant, only complete those portions of the Bid Sheet that are applicable. If bidding both positions, hourly rates must be provided for both positions. ETR reserves the right to accept or reject, and/or contract separately with an independent Mental Health Consultant, without consideration of the bid, unless bidder specifies award as "all or none."
