Description

THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. The Orlando Veteran Administration Medical Center contracting office is conducting a market survey to determine the availability of potential sources (and their size classification relative to NAICS 485991) having the skills and capabilities necessary to provide Non-Emergent Wheelchair and Stretcher Transport. The contractor shall provide all labor, material, equipment, vehicles, vehicle maintenance, documentation, reports, medical personnel and supervision necessary to provide wheelchair and stretcher service to eligible veterans from their residence to medical appointments at any of the corporate Orlando facilities and in certain circumstances, other VISN 8 Hospitals. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. No award will be made from this sources sought. Requested information should be submitted by email to oneil.sharpe@va.gov by 3:00pm EST November 26, 2012. Provide only the requested information below. STATEMENT OF CABABILITY: Submit a brief [five (5) pages or less] capability statement explaining your capability to perform non-emergent wheelchair and stretcher transport service for the Viera Outpatient Clinic. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your maximum capacity (maximum number of ambulance vehicles and wheelchair vehicles) and your availability to start. Please indicate the distance of your facility from the Orlando VAMC and surrounding Community Based Outpatient Clinics (CBOC). BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small. "If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business "If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (b) Indicate if your firm is a certified Hub zone firm (c) Indicate if your firm is a woman-owned or operated business (d) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (e) Include the DUNS number of your firm. (f) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry.
