- Under the authority of Public Law 104-262 and 38 USC 8153, the contractor agrees to provide Health Care Resources in accordance with the terms and conditions stated herein, to furnish to the Department of Veterans Affairs, in accordance with the services and prices specified herein. Contractor will provide Breast Imaging Services to the veteran patients at the Adam Benjamin, Jr. (ABJ) VA Outpatient Clinic in Crown Point, Indiana (a division of the Jesse Brown VA Medical Center, Chicago, Illinois). The services will include and are limited to mammography type procedures (screening, diagnostic, bilateral, unilateral, additional views), stereotactic image guided biopsy, computer aided detection technology, breast ultrasound, ultrasound guided breast biopsy/cyst aspiration, and associated cytology. All work performed under this contract will be completed at the contractor's facility. This contract is a non-personal services contract in accordance with FAR 37.401. 1. SCHEDULE OF ITEMS a. SOLICITATION NOTICE: This procurement is being conducted under Federal Acquisition Regulation (FAR) combination of Part 12, Acquisition of Commercial Items and Part 15, Negotiated Acquisition Procedures, and VAAR Part 873, Simplified Acquisition Procedures for Health-Care Resources. NOTE: As this is a Commercial Item Request for Quotation (RFQ) with price quotes and additional information as indicated in Addendum to 52.212-1. The above Schedule of Supplies/Services may not be in line with your commercial practice, you may provide an alternative proposal (*). If you are providing an alternative proposal, the Standard Form (SF) 1449 [pages one and two of the solicitation] and Clause 52.212-3, Offeror Representations and Certifications - Commercial Items, must be completed as well as written documentation for the alternative proposal. 1.General Contract Requirements: Breast Imaging Services are to be performed at the contractor's facility during the contractor's regular business hours. The facility will be located within 15 miles of the ABJ Outpatient Clinic. a)The contractor shall provide the services of an appropriately credentialed Radiologist. The contracting facility will have an accredited American College of Radiology (ACR) and Federal Drug Administration (FDA) inspected mammography program in good standing. Contractor will provide compliance documentation to the VA. b)An American Registry of Radiologic Technologists (ARRT) technologist will provide technical services. Documentation of license, continuing education and continuing experience is required. c)Equipment compliance and documentation performed by a licensed Medical physicist will be required for each mammography unit. d)Interpreting radiologist will meet ACR license, continuing education and continuing experience requirement. Documentation is required. e)The contractor will insure that staff provides high quality compassionate patient care for breast imaging procedures. This should be provided in a safe, sanitary and therapeutic environment. Patients should be treated with dignity and respect. f)Exams will be interpreted by the contractor and a verified final report faxed to designated number preferably within 24 hours of the exam but no later than 48 hours. g)Notification of results to the patient is to be communicated in accordance with Mammography Quality Standards Act (MQSA). h)Exam findings that are suspicious for significant abnormality biopsy recommended (BI-RAD 4) or highly suggestive of malignancy (BI-RAD 5) is to be communicated by the interpreting radiologist directly and immediately to VA designated physician/number. 2.Scheduling Exams: a)Appointments will be made by the VA with the contractor. Contractor will provide available appointment times. b)Contractor must be able to have appointments available within three weeks of request. c)Contractor will track and document no show, refusal of procedures or inability to contact patient; and, provide a report minimally on a monthly basis. d)Transportation is not the responsibility of the contractor. Patients or VA facility are to provide transportation to the contractor facility. 3. Image Archiving: Contractor shall retain mammograms for an indefinite period in accordance with Mammography Quality Standards Act (MQSA) regulations. 4. Film Release: Original mammograms, if requested, are to be released directly to the patient or to the VA Release of Information in accordance MQSA. 5. Exam Supplies: Contractor will provide all supplies needed for the listed exams and are fee inclusive. 6.Transportation: Transportation is not the responsibility of the contractor. Patients or VA facility are to provide transportation to the contractor facility. 7.Adverse Events: The contractor will provide to the VA data on 100% of adverse events associated with the care of VA patients on a quarterly basis.
