Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, VA260-13-Q-0126, is issued as a Request for Proposal (RFP). The North American Industry Classification System (NAICS) Code for this acquisition is 624190, Size Standard: $10 Mil. This procurement is open market no set aside restrictions. ITEM DESCRIPTION: Establish an Employee Assistance Program (EAP), Spokane VA Medical Center and Community Based Out-Patient Clinics in Wenatchee, Washington, and Coeur d' Alene, Idaho This solicitation will result in base year contract with 4 additional options Period of Performance is Dec 1, 2012 - Sept 30, 2017 Reference the attached RFQ for further details GRAND TOTAL --- ==================== FOB point is Destination: Department of Veterans Affairs, Spokane VAMC: Proposed effective date: Dec 1, 2012 Full text version of the clauses and provisions can be viewed at: http://www.acquisition.gov/far/index.html http://www.va.gov/oamm/oa/ars/policyreg/vaar/index.cfm The resulting firm fixed price award will incorporate the requirements of the clause Reference Attached RFQ The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, for the purposes of this document, the following blocks in paragraph (b) are considered checked: Reference Attached RFQ The provision at FAR 52.212-1, Instructions to Offeror--Commercial Items (June 2008), applies to this solicitation. Award will be made to the offeror representing the best value to the Government. All proposals will be evaluated in accordance with technical capability, compliance with the solicitation requirements and price. All Contractors, both large and small, are required to register in the SAM (system for Award Management) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to https://www.sam.gov/portal/public/SAM/ Quotations must be submitted using the attached RFQ. Quotations are due by Sept 20, 2012, 15:00 PM EST. The assigned Contracting Officer is Paul Richardson.
