Description

CONVERSION OF RADIO SYSTEM FROM WIDEBAND TO NARROWBAND BY JANUARY 1, 2013. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is CLA-13-Q-0008 for Conversion of the Claremore Indian Hospital's Motorola radio systems and equipment to Narrow banding 12.5 KHZ frequency. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. The NAIC code is 334419 and the small business size standard is 500 employees. This will be a single award December 1, 2012 or date of award through January 10, 2013. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. The clause found at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. FAR 52.212-1 Instructions to Offerors - Commercial applies to this acquisition. Offers are due November 26, 2012, 4:30 p.m. CST at Claremore Indian Hospital, Support Services, 101 South Moore Avenue, Claremore, Oklahoma 74017.
