- Amendment 0001 for W81K0012R0014. NOTE: This solicitation has been amended. See Amendment 0001 Document Attachment. The closing date has been changed to be Monday, 26 November 2012 at 3PM Central Time. All Documents to include past performance questionnaires must be received no later than 26 November 2012 at 3PM Central Time. Answers to all questions that were received by 31 October 2012 will be posted/uploaded on Tuesday, 13 November 2012. The Southern Regional Contracting Office has a Personal Services requirement for Various Healthcare Provider Services. This acquisition will be a Competitive 8(a) Multiple Award Indefinite Delivery/Indefinite Quantity Firm-Fixed Price Contract with task orders. This competitive 8(a) requirement will be set-aside 100% for the 8(a) program. Award will be made to more than one 8(a) vendor. The primary locations of the services to be performed are the San Antonio Military Health System (which includes all Military Treatment Facilities and clinics within the San Antonio, Texas area), Carl R. Darnall Army Medical Center (includes all clinics on and off post in the Fort Hood, Texas area), and Bayne-Jones Army Community Hospital (Fort Polk, Louisiana), but may include other military medical treatment facilities that fall within the area of responsibility of the Southern Regional Contracting Office. The types of medical specialties and the number of health care providers will vary depending on the needs of each MTF, but will include services of both physicians and ancillary health care providers. The base period of performance is on or before 1 April 2013 through 30 September 2013, plus four (4) twelve-month option periods (through 30 September 2017). This requirement will be solicited as a Competitive 8(a) Program Set-Aside. The North American Industrial Classification System (NAICS) code is 622110, General Medical and Surgical Hospitals. The Small Business Administration's size standard for NAICS 622110 is $34.5 million. The solicitation number is W81K00-12-R-0014. The solicitation will be available on/about 17 October 2012 at the Army Single Face to Industry (ASFI) website (https://acquisition.army.mil/asfi/solicitation_search_form.cfm) and FedBizOpps (www.fbo.gov). Prospective offerors are advised that if tentatively selected for award, they must register with the Central Contractor Registration database (www.ccr.gov) and obtain a Commercial and Government Entity (CAGE) code prior to any award being made. Award will be made on/about 31 December 2012. Paper copies of this solicitation will not be issued, and telephone and faxed requests for the solicitation will not be accepted.
