Sources Sought Notice The Naval Hospital, Camp Lejeune NC respectfully requests sources sought for the service listed below. Responses should be returned via e-mail by 10 AM EST on November 16, 2012. Please submit responses to the following email addresses: Summer.griddine@med.navy.mil Corey.Jenkins@med.navy.mil Anticipated POP: 03 December 2012- 02 December 2013 Please submit price quotes in the Firm Fixed Price (FFP) format. CLIN 0001 Bus Driver Support INTRODUCTION This requirement is located in the Laboratory/Clinical Pathology Department, DOD Military Blood Program, Naval Hospital, Camp Lejeune. Primary purpose is to operate gasoline and diesel vehicles, from a 15 passenger van to a 45' Class A RV-style Blood Mobile. Incumbent may also be required to operate: sedans; 5, 8, 12, & 15 passenger vans; ambulances; pick up trucks; 2 and 5 ton trucks with hydraulic lift gate; 36 to 45 passenger buses or larger; a Bloodmobile with seating for up to 15 passengers; an audio truck; and any other vehicle required to perform transportation duties. TYPICAL WORK PERFORMED • Incumbent operates gasoline, diesel, propane or other fueled vehicles weighing up to 35,000 pounds GVWR. These vehicles may include enclosed vans, stake trucks, buses, and special patient or handicapped personnel transporting vehicles, ambulances, bloodmobile, audiology testing vehicle and forklifts up to and.including 15,000 pounds. These vehicles have up to five forward speeds and are equipped with driving controls such as shift levers, brake pedals, accelerators and clutches (when vehicles are equipped with manual transmissions) and levers for supplying power to the driving wheels. This task is 50% of the required services • Incumbent drives vehicles either on a specific trip basis or upon a regularly established schedule and route to transport material, equipment, and personnel. Driving assignments may be to local or long distance destinations. This task is 25% of the required services • Incumbent loads and unloads materials/equipment or directs laborers in this task. Checks material loaded or delivered against records of various types to ensure against errors, and obtains signatures upon delivery where required. Keeps trip records or logs, showing time of departure, sequence of stops, time of return, trip mileage, fuel and oil consumption, and similar items. Reports any defects in vehicle operations. Reports accidents, damages or traffic violations using Standard Form 91. If incidents occur on base, incumbent must inform base police and the Transportation Supervisor. If such occurs off base, must phone local police or Highway Patrol. This task is 10% of the required services • Incumbent must make operational inspections of vehicles for loose parts, damaged or worn tires, proper tire air pressure, adequate fuel supply, radiator fluid level, proper oil level, battery condition, and similar Items. Must ensure assigned equipment, such as tools and fire extinguisher, are In vehicle before starting. Cleans assigned vehicles or supervises laborers This task is 10% of the required services • Incumbent may be asked to provide transportation services for other departments of the Naval Hospital. Tasks can include data entry, word processing, filling out forms, assigning vehicles, sending/receiving fax, and dispatching vehicles and drivers. This task is 5% of the required services Perform other related duties as assigned. FACTOR STATEMENTS A. SKILLS AND KNOWLEDGE Incumbent must be skilled in the use of controls for starting, stopping, backing and driving assigned vehicles: Because of the height, width, length, and weight of the vehicle, he/she must be able to make sound judgments concerning loading, arranging cargo, clearances, turning radius, braking distance, and distance to maintain from a preceding vehicle. Must be able to recognize hazardous conditions and avoid such. Because of the weight and size of vehicle of some vehicles, and laws restricting use of some roads to certain sizes of vehicle, must be selective in choice of routes. Must be able to use a two-way radio. Incumbent must have the ability to read and write English and interpret road maps and work instructions. Must have knowledge of arithmetic to prevent the overloading of vehicles and to determine width, length, and/or height limitations. Incumbent must possess a current North Carolina Class B Commercial Driver's License with P endorsements. Must maintain current medical examiner's certificate, (DL 51A). Incumbent should have some experience with operating a desktop computer and knowledge of simple database, spreadsheet and word processing-programs. B. RESPONSIBILITY Incumbent is under the supervision of the Head, Blood Donor Program Department. Incumbent is responsible for proper operation of vehicles assigned to prevent injury to others and damage to property. Is responsible to obey all traffic laws and safety regulations. Is responsible for following specific route and arriving at specified time. If assigned to do so. Otherwise is responsible for selecting routes. If incumbent is not able to follow specific route and arrive at specified time, incumbent must notify the Supervisor as soon as possible. Is also responsible• for loading, unloading, and arranging of cargo. Incumbent must accomplish assignments with considerable independence and is responsible for the safe operation of vehicles and equipment. Work is evaluated in terms of customer satisfaction, timeliness and driving competence. C. PHYSICAL EFFORTS Reaches, bends, turn, moves hands, arms, feet and legs to operate the various hand and foot controls. Lifts heavy chains, hooks, etc., to make coupling between vehicle and towed equipment. Loads and unloads vehicles. Heaviest weights handled average approximately 50 pounds. Objects over 50 pounds are handled with the assistance of other personnel or mechanical aides. D. WORKING CONDITIONS The incumbent works outdoors about 90% of the time. Occasionally exposed to bad weather conditions while loading and unloading cargo. Drives in all types of weather conditions. Also, drives In all types of traffic conditions upon all types of roads. May be exposed to the possibility of serious accidents. ***Incumbent may be required to work rotational shifts. Incumbent may be required to work weekends and holidays.
