Description
Infant & Toddler Kit List Complete RFQ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. HSFE70-13-Q-0012 is issued as a Request for Quotations (RFQ). HSFE70-13-Q-0012 and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-61. This requirement is for a firm-fixed price contract. Award will be based on the lowest price technically acceptable. Responses to the combined synopsis/solicitation due date is 8:00 AM EST 6 NOV 2012. Responses should be emailed to Amanda.Rogers@fema.dhs.gov. FEMA is requesting up to 100 completed Infant & Toddler Kits as outlined in the below description of work and attached Kit List. All perishable items will have a minimum nine (9) month shelf life at time of delivery. Vendors shall provide a quote with their proposed kit unit price, estimated shipping charges, and earliest delivery date. DESCRIPTION OF WORK: • Assemble the Infant/Toddler Kit components and deliver to a FEMA designated location. The Contractor(s) shall assemble the kits at the Contractor(s) location and deliver full, complete kits to the designated location. The Contractor(s), using their own equipment, shall inventory all materials and prepare an excel spreadsheet that compiles the perishable and non-perishable components of the kits. For the perishable items; the spreadsheet should capture data regarding the manufacturer, item description, quantity, expiration date(s) and lot numbers. The Contractor(s) shall email the Contract Specialist a copy of the spreadsheet once the kits have shipped. PACKAGING/LABELING: • Each kit shall contain a packing slip that details the component, quantity and shelf life information. One (1) copy shall be attached to the outer shell of the kit and one (1) on the inside. *Shelf life data includes manufacturer, lot number & expiration/use by date. • Each kit will be packaged in a tri-wall container (i.e. 48 x 40 x 36" ECT 51 Double Wall Easy Loader Box Kits) with the perishable items segregated from the non-perishables. The tri-wall will be labeled to identify the kit number, contents, quantity of each item and shelf life information. The tri-wall shall be on a 40x48 four way pallet, stacked no higher than 55" and shrink wrapped no less than 10 times to secure the kit to the pallet. Label Example: FEMA Infant/Toddler Kit # XXXX Item # Product Description Manufacturer Quantity Lot Number Expiration Date 001 Baby Food (meat) XYZ Corp. 75 xxx MM/DD/YYYY SHIPPING/DELIVERY INSTRUCTIONS: All kits will be shipped to a FEMA designated location in either Lake Hurst, NJ or Farmingdale, NY within 48 hours of award. Destination information will be provided at award. Vendors should anticipate that the delivery trailer will have to be dropped and detained for up to 30 days. PLEASE SEE ATTACHED DOCUMENTS.
