Description
This is a request for quote for Airport Traffic Control Tower Facility roof system replacement at Richard Lloyd Jones Jr. Airport, Tulsa, OK. Vendor will provide all materials, supervision, labor and equipment to perform all work according to the attached Scope of Work. Site is located in Tulsa County, Oklahoma. Projected start date is 30 days after receipt of award. The FAA will not pay for any information received or costs incurred in preparing the response to the request for quote. Therefore any cost associated with the request for quote submission is solely at the interested vendor's expense. A SITE VISIT WILL BE CONDUCTED 10:00 AM CST, TUESDAY, NOVEMBER 6, 2012, AT THE AIR TRAFFIC CONTROL TOWER (ATCT), 8801 SOUTH JACK BATES AVENUE, TULSA, OK 74132. CONTACT JAMES BLAKESLEY, 918-948-4916, james.m.blakesley@faa.gov, TO HAVE YOUR REPRESENTATIVE(S) NAME ADDED TO THE ACCESS ROSTER. THIS IS THE ONLY SITE VISIT THAT WILL BE HELD. The competition will be based on lowest price and past experience. This competition is open only for small businesses, or service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SBA for participation in the SBA's 8(a) Program. Contractors must be registered in the System for Award Management (SAM) database prior to submitting a response. Vendors participating in the SBA's 8(a) Program must provide a copy of their SBA 8(a) certification letter. In order to make this determination the FAA requires the following from interested vendors, which will be used in the evaluation process: 1. Pricing. 2. Past experience - This document should identify: Size and type of services provided by previous contracts (elaborate and provide detailed information, and past performance), Contract numbers, points of contact information, Number of years in business Attached are the Specifications, drawings, Terms and Conditions, and a Business Declaration Form. Please respond by reviewing these items, and submitting a completed, signed pricing sheet, past experience information and a filled-out and signed copy of the Declaration form (please submit electronically, email to address below). All responses to this request for quote must be received by FRIDAY, NOVEMBER 16, 2012, 2:00 PM CST (Oklahoma Time).
