Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit bids. Solicitation VA-249-13-R-0057 is issued as a Request for Proposal (RFP) and constitutes the only solicitation, a written solicitation will not be issued. This procurement will be a small business set-aside. NAICS Code is 561730 and size standard is $7.0 million employees. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all service, labor, and maintenance of procurement requirements listed in the attached statement of work. This service is required for the Tennessee Valley Healthcare System VA Medical Center, 1310 24th Avenue South, Nashville, Tennessee within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a proposal on the base year and all option years as provided below. Submitting Proposal: Contractor shall submit their proposal on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, overall total price for base year and all option years, and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document. Please no phone calls. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following evaluation factors and any significant sub factors that establish the requirements of acceptability shall be used to evaluate offers. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability criteria for non-cost factors. EVALUATION CRITERIA: 1)TECHNICAL CAPABILITY/EXPERIENCE a.Offeror shall provide a complete background of their company including their full legal name, primary business, address, all other office locations (city and state) and history along with the number of years in business. b.The contractor shall provide a list of degreed horticulturists or experienced landscape maintenance professional to supervise the application of chemicals and manage the landscape maintenance program in its entirety. c.The contractor shall provide a qualified on-site superintendent to insure high quality work. The superintendent is required to be on site at all times during the performance of the work. d.The contractor is responsible for providing a licensed chemical operator capable of incorporating EPA approved materials and application methods. (2) PAST PERFORMANCE a.The Offeror shall provide a listing of all contracts performed within the past three years that are similar in nature and relevant to the efforts required by this solicitation no later than 11/14/12. The Offeror shall furnish the following: (a) Customer (Government or Commercial) and point of contact to include name, address, telephone number, fax number and email address for each contact, (b) Service(s) provided including nature of contract, dollar value, etc., (c) Description or relevance and similarity to the effort required by this solicitation. Offeror will be evaluated on customer service, timeliness of performance, quality of services provided. (3) COST/PRICE: a.Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability criteria for non-cost factors. NOTE: The Government reserves the right to award without discussions; therefore submit your most favorable terms with your proposal. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
