Description
Request for Information VA-797H-13-I-0009 Department of Veteran Affairs, National Acquisition Center, National Contract Service - High Tech Medical Equipment High Tech Medical Imaging Equipment I. Information/Requirements: The Department of Veteran Affairs is seeking responses to this Request for Information (RFI) from interested, capable, small business firms that can provide a full spectrum of High Tech Medical Imaging Equipment that will be identified below. These responses will assist the Department of Veterans Affairs (VA) in performing market research, in accordance with Federal Acquisition Regulation (FAR) Part 10.002. This RFI is only for information and planning purposes that may result in multiple awards and shall not be construed as a solicitation or as an obligation on the part of VA. The applicable North American Industry Classification System (NAICS) code is 334517. VA is currently conducting market research to locate qualified, experienced and interested potential sources. Responses will not be considered as quotes, nor will any award be incurred by interested parties responding to this sources sought announcement. The purpose of this RFI is to seek interested sources (Vendors) and for near-future solicitation(s) and procurement(s) for High Tech Medical Imaging Equipment. This RFI seeks to identify small businesses that are capable and interested in being able to provide all the following High Tech Medical Imaging Equipment: A.COMPUTED TOMOGRAPHY (CT) SCANNERS, B.MAGNETIC RESONANCE IMAGING (MRI) SYSTEMS C.X-RAY SYSTEMS D.NUCLEAR MEDICINE E.ULTRASOUND F.COMPUTED RADIOLOGY (CR) G.MAGNETOENCEPHALOGRAPHY H.PET SCANNER I.CYCLOTRON J.RADIOGRAPHY SYSTEMS K.X-RAY FILM LASER IMAGERS High Tech Medical Imaging Equipment vendors/manufacturers requirements include but are not limited to providing all the following upgrades and accessories, including installation and leasing of radiology systems and equipment of: A.CT B.MRI C.X-RAY D.NUCLEAR MEDICINE E.ULTRASOUND F.COMPUTED RADIOLOGY G.AGNETOENCEPHALOGRAPHY H.PET SCANNERS, CYCLOTRON I.RADIOGRAPHY SYSTEMS. High Tech Medical Imaging Equipment vendors/manufacturers requirements include but are not limited to providing training on all the following items for customers: A. CT B.MRI C.X-RAY D.NUCLEAR MEDICINE E.ULTRASOUND F.COMPUTED RADIOLOGY G.AGNETOENCEPHALOGRAPHY H.PET SCANNERS, CYCLOTRON I.RADIOGRAPHY SYSTEMS High Tech Medical Imaging Equipment vendors/manufacturers requirements include but are not limited to providing parts/accessories on all of the following items for the course of ten years after the delivery and acceptance of the following items: A. CT B.MRI C.X-RAY D.NUCLEAR MEDICINE E.ULTRASOUND F.COMPUTED RADIOLOGY G.AGNETOENCEPHALOGRAPHY H.PET SCANNERS, CYCLOTRON I.RADIOGRAPHY SYSTEMS II. Submittal Information: Vendors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 10 pages in length, single spaced, 12 point font minimum that addresses the above information. The Government will not review any other data or attachments that are in excess to the 10 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. A.Capabilities / Qualifications 1.A narrative summary that describes the capabilities and qualifications to perform these functional task areas. 2.Company Information: a.Company Name b.Address c.Point-of-contact name, phone number, and e-mail address d.Dunn & Bradstreet number e.Business size, f.Socioeconomic status, g.Number of years in business, h.Any General Services Administration (GSA) Federal Supply Schedule (FSS) contract numbers and relevant Special Item Numbers (SINs) and/or other Government contracts (if applicable) It is requested that the above information be provided no later than October 23, 2012 12:00PM CST.
