Description

Statement of Work GSA Authorization Letter Tax Exempt Letter CSC Vendor Forms 1 job - Contractor will furnish all plant, labor, materials and equipment in performing all operations necessary to remove existing rubber surface, volleyball post mount, glue and joint seal material from all areas which currently have rubber surface, volleyball post mount, glue and joint seal material and replace with new painted surface materials. (See SOW for complete details.) Please fill out,date and sign the attached CSC Vendor Certification package. Corporate requires documents to be filled out prior to any award. MSDS material form and attach a copy of your certificate of liability insurance. Must fill out and return background checks on all employees that will or may be working on Vance Air Force Base premisises two weeks prior to beginning the project. CSC Applied Technologies LLC requests best pricing and any additional discounts that may be available either advertised or unadvertised in response to this request for quote. The vendor is requested to advise of any additional discounts or if none, so state, via a reply E-mail with subject "Discounts". Pre-Award visit will be on October 22, 2012 at 2:00 p.m. Central. Let Lydia Johnson know if you will be attending so that prior arrangements can be made for base access. After Soliciation Issue it is Your Responsibility to Check the FBO site Frequently for possible Addenda or other Items that may affect the soliciation and the acceptance of your bid.
