This is a combined synopsis/solicitation for commercial items under FAR part 12 USING simplified acquisitions in FAR part 13. This announcement constitutes only the solicitation; quotations are being requested and a written solicitation will not be issued. This is a total small business set-aside. The NAICS code is 621112. Johnson VA Medical Center 1 Medical Center Drive Clarksburg, WV. 26301 6.Wilmington - Wilmington VA Medical Center 1601 Kirkwood Highway Wilmington, DE. 19805 The provisions and clauses in Attachment E are included in this solicitation and the clauses will be part of the awarded contract. Attachments B and C are in the same pdf file attachment labeled Attachments B & C. Psych Assess Memo.pdf. Attachment B consists of pages 1 -20 of the pdf file attachment. Attachment C consists of pages 21 - 26 of the pdf file attachment. Instructions for Submitting Your Quote If you decide to submit a quotation in response to this Request for Quote (RFQ), please ensure that you have read all documents. Quotes must include the completed Past Performance/Reference document (Attachment D), Schedule of Supplies and Services document (Attachment I). All questions and quotes are to be emailed to Mark.Anderson7@va.gov and no questions will be accepted after October 11, 2012 at noon unless Contracting Officer amends this solicitation. It is in the government's best interest to review this solicitation based on a Best Value decision. Quotes that are overly verbose include marketing material, or non-essential graphics may distract from the evaluators ability to ascertain compliance with the RFQ and are strongly discouraged and may be removed before sending forward for technical evaluation. Quotations shall include the solicitation number (VA244-13-Q-0015), the proposed unit price, the proposed extended price, any prompt payment discount terms, company name, point of contact, address, phone number, fax number, tax identification number, DUNs number, and size status (i.e., small business, disadvantaged business, women owned, HUBZone, etc). All sources must submit a quote with required Offeror Representations and Certifications by October 15, 2012 at 1700 (5:00 pm) Eastern Standard Time. Quotes received after that time may not be considered. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and is determined by the Contracting Officer to be most advantageous to the Government in accordance with FAR 13.106. More information regarding the evaluation procedures being used can be found in Attachment H, labeled Attachment H. Evaluation Criteria.
