- Pre-Solicitation for #VA69D-12-B-1198 Project Number: 695-11-115 Project Title: 111 Upgrade Prosthetics (Product Line Improvements) Project Location: Zablocki VA Medical Center in Milwaukee, WI Project Description Contractor shall provide all tools, materials, components, labor and supervision to complete the construction of Project 695-11-115 111 Upgrade Prosthetics whereby work shall include renovating exiting office space presently located in the Southeast corner of the Basement level in Building 111 at the Clement J. Zablocki VA Medical Center in Milwaukee, WI. This project consists of three (3) major elements as listed below: 1)Cube City consists of building out the new basement space under the kitchen to include all architectural, mechanical, equipment, furnishings, and electrical work. Then the existing Cube City furniture and equipment must be moved to the new space. As part of the new space additional modular furniture as required. 2)Prosthetics consists of relocating existing Corridor COA6, Administrative Offices and Work Stations, Kitchenette, Wheelchair Repair and Washing, Prosthetics Parts Inventory Storage, Prosthetics area and Workshop, Orthopedic Workshop, Amputee Clinic, Exam Rooms, Respiratory Therapy room, Reception, Waiting and Training Room. Construction will include the complete demolition, removal and replacement of all asbestos insulated piping/ductwork/flooring/ceiling, duct insulation, floor tile, etc., architectural, mechanical, equipment, furnishings, and electrical work. 3) OT and PT consists of combined OT/PT Charting, Staff Breakroom, OT Supervisor's Office, Biofeedback room, Ergonomics room, OT Fitness room, OT/PT Storage room, Male and Female Public Bathrooms, elimination of corridor COB1, rooms B0301 and B0311 to create a combined OT/PT Therapy room. Construction will include the complete demolition, removal and replacement of all asbestos insulated piping/ductwork/flooring/ceiling, duct insulation, floor tile, etc., architectural, mechanical, equipment, furnishings, and electrical work. In accordance with Public Law 109-461, the Veterans Benefits, Healthcare and Information Technology Act of 2006, Pursuant to 38 USC 8127(d), competition is 100% Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB) firms. The SDVOSB must be currently registered and verified by the Center for Veterans Enterprise, (CVE) at the time of the bid deadline. To be eligible for award, vendor registration information must be most-current/updated on the following web-sites: http://www.vip.vetbiz.gov, http://ccr.gov, and http://orca.bpn.gov. The solicitation will be issued on/about August 21, 2012 with the bid opening on/about September 19, 2012. Dissemination of the solicitation is limited to electronic medium. All interested Bidders (primes & subcontractors) should register at www.fedbizopps.gov so that others will know of your interest in participating in this procurement and for automatic receipt of updates or amendments. Amendments to the solicitation will be posted at www.fedbizopps.gov. By registering to "Receive Notification", you will be notified by e-mail of any new amendments that have been issued and posted. Bidders are reminded that they are responsible for obtaining and acknowledging all amendments to this solicitation prior to the time for receipt of offers. All requests for information are required to be in writing to the Contract Specialist assigned to this Solicitation at kristi.kluck3@va.gov; telephone inquiries will NOT be accepted. Award is subject to the availability of funds. There will be a pre-bid walkthrough held for this project and all prospective bidders are encouraged to attend. Details of the pre-bid walkthrough will be included in the solicitation package. The period of performance for this contract is 420 calendar days after notice to proceed has been issued. The project magnitude range is between $1,000,000 and $2,000,000. The North American Industry Classification Code (NAICS) for this procurement is 238140; size standard $14.0 million applies.
